Atlantic Behavioral Health is an IOP Behavioral health practice in Wilmington, MA.

This is a M-F in-office position.
Previous experience with mental health/behavioral health treatment and case management strongly preferred.
Traditional case management functions will include but not be limited to:
· Developing the client discharge/transfer plans to ensure movement to the appropriate levels of care.
· Contacting referral sources, providing case consultation and coordination with referral sources (for example: mental health treatment providers, criminal justice agencies, schools, employers, Child Protection Services)
· Coordinating ongoing care for client housing and therapeutic needs with the Back Bay Treatment team, substance abuse treatment providers, healthcare providers, community resources, and family members
· Participating as a member of a multidisciplinary team for assigned clients
· Documenting all counseling and education services, assessments, reassessments, referrals and follow-up in the client's clinical record, providing appropriate signatures and dating of such entries, including those made in electronic records
· Positively promoting Atlantic Treatment to prospective clients and employees
· Orientating new clients to the facility and the PHP/IOP facility
· Facilitating communication with ancillary services
· Attending clinical and administrative meetings to integrate inter-departmental sharing of information
· Building positive relationships with clients
· Demonstrating adaptability and flexibility to changes in the work environment
· Following Infection Control Guidelines at all times
· Reporting personal symptoms of suspected illness or contagious diseases to the Infection Control
· Providing a safe, therapeutic environment for staff and clients.
· Case Managers may also perform the work of a Counselor including:
1. Obtaining previous records that are relevant to the current treatment episode
2. Providing active case consultation
3. Facilitating groups
