Summary:
Provides intensive community-based case management and crisis intervention services to children with significant behavioral health challenges and their families in the Child & Family Therapeutic Outreach program by performing the duties that follow.
Responsibilities:
MAIN DUTIES AND RESPONSIBILITIES:
Provides case management and crisis intervention services to children at imminent risk of hospitalization or disruption to a placement and their families. Includes but is not limited to:
Face-to-face contact with children and their families in their own homes or community as prescribed in the Treatment Plan.
Coordination of services through documented meetings documented telephone conversations and correspondence.
Under the guidance of the primary Clinicians provides coordinates and monitors treatment services for all assigned cases. Includes but is not limited to:
Face-to-face and telephone contact with client documented in progress notes following established procedures as prescribed in the Treatment Plan.
Provision and coordination of care prescribed in the Treatment Plan through documented contact and implementation of assigned activities with client/client�s family
Provides coaching to families in use of prescribed interventions that have been effective with the child following treatment guidelines.
Is alert and responsive to client issues and behaviors; works within prescriptive instructions from the primary Clinician and the Treatment Plan; modifies treatment to meet client needs and changes in situation.
When providing services arrives on time and participates as scheduled.
Provides psycho-educational services to children and their families based on modality frequency participant and location specifications prescribed in the Treatment Plan and as determined by the Clinician and/or Program Manager. Services mayll include provision of information regarding diagnosis social/emotional development mental illness substance abuse effective parenting behavior management etc.
Maintains current knowledge of treatment strategies that are effective for the population; uses and seeks out information to improve competency in intervening in behavioral health problems in the population and in supporting families.
Attends Discharge Planning meetings at inpatient and partial hospital facilities as requested.
Documents with client when possible progress notes according to procedures and timeline.
Functions as liaison and client advocate with community agencies from which a client is receiving services including but not limited to:
- Attending community agency meetings.
- Reporting issues raised at liaison and community meetings to primary Clinician and Supervisor.
- Making arrangements with other agencies to provide services for identified clients as determined with the primary Clinician.
Maintains a minimum of 23 direct service productivity hours per week for a full time position.
Participates on committees as designated by supervisor attending meetings on time and actively participating in both discussions and assignments.
Participates in other inter-agency meetings and trainings as designated by supervisor attending meetings and trainings on time and actively participating in both discussions and assignments.
Provides public outreach services as requested.
Prepares for and attends individual and group supervision meetings and follow through on supervisory guidance and instruction.
Performs other duties and responsibilities as assigned or needed.

QUALIFICATION REQUIREMENTS:
Bachelor�s Degree preferably in related psychology education or mental health field.
Experience working with children who have behavioral health disorders.
NECESSARY SPECIAL REQUIREMENTS:
Valid driver�s license
Insured automobile
Ability to work a flexible schedule when clients are available after school
JOB KNOWLEDGE SKILL AND ABILITY:
Knowledge of neuro-typical development characteristics of children with varying developmental and behavioral health disorders and family systems issues.
Ability to communicate effectively both in writing and verbally with clients family members supervisors other agency staff community professionals and other members of the community.
Ability to effectively present information and respond to questions from professionals managers family and the general public.
Advocacy and teaching skills.
Ability to successfully complete all required training and certifications
Oral and written communication skills including speech grammar spelling and legibility are at sufficient level to ensure effective and appropriate communication.
Ability to add subtract multiply and divide in all units of measure using whole numbers common fractions and decimals either manually or by use of a calculator.
