Job Description:
OVERALL OBJECTIVE OF JOB: To provide rental assistance and coordinate support for homeless and near homeless individuals and move families towards self-sufficiency.
ESSENTIAL FUNCTIONS OF JOB:
- Conducts preliminary interviews and intake to identify needs of the clients and families.
- Provide information, assistance and referral services, including the completion of applications and forms, for Aging and Community Services.
- Works one-to one with clients to aid in providing community services to the client and determine eligibility for the rental assistance program.
- Develops a personal service plan to best meet client’s needs and provide client(s) with self-sufficiency, financial management, and other training as needed.
- Advocates the interests and needs of the client with other departments, government entities, community organizations and/or social service agencies.
- Maintains continued contact with the client to ensure client’s needs are met within a two-year period.
- Prepare and submit rental assistance invoices for Fiscal.
- Prepares monthly, quarterly, and yearly reports for HAP and HUD.
- Establishes and maintains appropriate filing and record keeping systems.
- Maintains an open communication with landlords for collaboration.
- Maintains an open communication with referral resources and agencies (with appropriate release of information agreement signed by client).
- Protects the confidentially of the client(s).
- Makes home visits to clients as necessary (when all other modes of transportation have been exhausted) and with permission from supervisor.
- Maintain required clearances and education, including Child and State, and Federal if needed.
OTHER DUTIES OF JOB:
- Utilizes computer, telephone and other office equipment as needed to perform essential job functions.
- Trained for appropriate applications and systems.
- Attends meetings, training and conferences as required.
- Assists other staff as required.
- Performs other job-related functions as required.
Minimum Education and Training:
Six months of experience as a Human Services Case Manager, or County Human Services Intern; or one year of experience in public or private social work and a Bachelor's Degree which includes or is supplemented by 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social sciences; or a bachelor's degree with a social welfare major; or any equivalent combination of experience and training including successful completion of 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social sciences.
