About the Position:
CCM (Chronic Care Management) requires the candidate to review the patient's medical chart, to monitor a patient's progress with his/her treatment plans, review the patient's compliance to our clinic's preventative care protocol, provide patient education regarding the patient's chronic condition(s), and to build a strong rapport with the patient. The successful candidate is passionate about patient care, passionate about patient education, and passionate and creative in influencing the patient to make small and big changes in their lifestyle to maintain their optimal health.

Schedule:
- 8-hour shift
- Day shift
- Monday to Friday
- $20.00 – $21.00 per hour depending on experience

- In person in Folsom or Fair Oaks, TBD.

Attributes of a Strong Candidate:
- High school diploma.
- Graduation Certificate as Medical Assistant.
- Strong communication skills and the ability to work well with a team in a fast-paced environment
- Previous clinical experience is preferred, but willing to train the right candidate.
- Must have excellent grammar, spelling and punctuation.
- Epic software experience preferred.
- Knowledge of Apple and Android phones to assist with set up, and tracking is preferred.
