Overview:
We are looking for a Centralized Bereavement Coordinator Manager to join our team. This position will directly report to the Vice President of Operations and is responsible for participating with other members of the interdisciplinary team in multiple branches to identify the bereavement needs of the patient and family.
- Define goals which address individual needs and provide the highest quality service to meet those needs
- Oversee the day-to-day operations of the Centralized Bereavement Team
- Assist with overflow calls, train, approve time and PTO requests and manage performance of the team
- High school diploma or equivalent; undergraduate degree in human service field preferred
- Counseling responsibilities require graduate degree in a related discipline
- One year required, three years preferred, clinical experience in a healthcare setting; preferably hospice, home health, community care or hospital
- Three years or more of previous team leadership experience
- Evidence of training in the areas of life-threatening and chronic illness, grief, and loss
- Clinical experience in the areas of life-threatening and chronic illness, as well as grief and loss
- Knowledge of terminally ill patients and their families along with understanding of hospice and the psychosocial dynamics of illness, loss, and death
