About the Job
Under the direction of the RHC Operations Manager, the SNF Certified Medical Assistant is responsible for assisting in the delivery of health care and patient care management for long-term care patients. Ensures effective communication about patient care needs with SNFology providers and long-term care facilities. Primary point of contact with SNFology providers for scheduling and administrative needs.
Essential Duties and Responsibilities:
- Perform administrative duties such as answering phones, scheduling appointments, and maintaining medical records
- Confer with SNFology providers as needed for daily SNFology appointments and patient needs, including any incomplete patient testes/consults or other incomplete orders prior to patient visit
- Complete all necessary documentation to schedule, register, and complete intakes on daily appointments
- Fax completed chart notes to long-term care facilities
- Complete medication reconciliation and process RX refill requests for providers
- Facilitate communication on patient care needs between provider and long-term care facilities.
- Triage and respond to patient communications within practice policy
- Track monthly rounding schedule for providers at all facilities
- Track new admissions to SNFology practice
- Maintain communication on new admissions to SNFology practice
- Create/maintain spreadsheets to track data necessary for regulatory/audit purposes, including Consent for Treatment/HIPPA forms
- Communicate with long-term care facilities for missing admission documentation
- Scan all required paperwork into patients’ charts
- Review faxes on fax machine, notify providers if urgent issues
- Create/maintain spreadsheet for new admissions/schedules for SNFology providers
- Maintain effective communication with SNFology team members and program supervisors
- Verify insurance as necessary for billing purposes
- Review and respond to correspondence for practice/providers
- Process pharmacy requests for medical equipment (DME), including but not limited to amending chart note, obtaining provider approval/signature, process necessary paperwork (as needed)
- Complete medication prior authorizations
- Communicate with provider/staff on daily Teams Chat
- Perform selected nursing (taking vitals) and administrative duties and assist the provider as part of the clinical care team
- Conduct pre-visit planning by reviewing and updating patient charts prior to appointment with recent test results and correspondence and determines needed services per practice protocol
- Coordinate care with disease management or case programs as needed.
- Assist with scheduling tests and treatments, obtaining pre-certification if necessary, for hospital care and/or procedures for patients, and process referrals to other providers.

Job Qualification Requirements:
Education: Graduate of an accredited Medical Assistant Program
Licensure/Certification: Certification as Medical Assistant required, as CMA, RMA, NCMA, CCMA or CMAA. CPR certification required within one month of hire.
Experience: One to three years of clinical experience preferred. Long term care experience preferred. Ability to maintain strict confidentiality. Knowledge of Microsoft Word and Excel preferred. Knowledge of admissions protocols and procedures within an LTC/SNF environment.
Physical Requirements: Frequent sitting and occasional periods of standing/walking. Long periods of data entry. Occasional bending, stooping, and reaching. Occasionally lift up to 25 lbs.
