Responsibilities:The Certified Case Counselor – Supervisor (CCC-MCM) is responsible for overseeing the administrative needs of hospitals or health care facilities, specifically the case management team of the Peter Ho Memorial Clinic. The CCC-MCM plans schedules and manage the staff, supplies, and work distribution of the case management team. The responsibilities include screening/interviewing prospective staff, training new staff, and scheduling their shifts. The CCC-MCM provides guidance and assistance in assessing medical, insurance, housing, social services, community, legal, financial, and other services to the patients of the Peter Ho Memorial clinic. The CCC-MCM will also support the case management team with completion of intakes, psychosocial assessments, development, and re-evaluation of individualized client service plans and follow up on referrals as often needed. Maintains documentation in the EPIC, NJDOH system (CareWare), and Ryan White Grant reporting systems (CHAMPS), ensure accurate & prompt data entry of service deliverables of the case management team. The CCC-MCM educates and consults with service providers and consumers concerning HIV disease and the process of meeting consumers’ identified needs. CCC-MCM responsibility also includes client-specific advocacy and review of use of services, including face-to-face, telephone, and other forms of communication.
Qualifications:EDUCATION, EXPERIENCE, TRAINING
1. Minimum of five (5) years’ experience. Working in case management, discharge planning or other health related field preferred. Working with persons with or at high risk of HIV infection preferred.
2. Master Level degree required, Master’s in Social Work, psychology, or other health care related field preferred.
3. Prior supervisory experience is preferred.
4. Ryan White Case Management Certificate obtained within 6 months of hire required.
5. Experience and knowledge in basic to intermediate computer skills (Microsoft Office: Word, Excel, PowerPoints, email, etc.). MCM training and Behavioral Violence Prevention Training within 3 months of hire and maintain current.
