Certified Case Manager - Adult Team - Weber Human Services

Weber Human Services (WHS), located in Ogden, Utah serves a growing urban population residing in Weber and Morgan Counties.

We are an organization dedicated to helping those in need rebuild their lives and reach their full potential. WHS was organized by a cooperative agreement between the Weber and Morgan county governments to serve as the local authority for the provision of their substance use, mental health and senior service programs.
Weber Human Services is the only organization in the State of Utah providing these services under a single administration. Through dozens of human services programs, WHS helps thousands of its neighbors by supporting and empowering our most vulnerable groups including at-risk youth, the elderly, children, youth and adults suffering from mental illnesses, and those recovering from addictions.

Provide monitoring, linking, coordinating, advocating, outreach, and other medically necessary services to clients. Coordinate with Adult Team members and other community service providers to enhance quality of life and avoid exacerbations of illness and/or hospitalization.

Teach daily living skills focusing on recovery and increasing the client's ability to care for themselves in the least restrictive environment through individual and/or group home development.

Focus on Critical Time Intervention and coordination with the inpatient unit and treatment team to help maintain the client's stability outpatient.

Documenting services accurately, professionally, and within the treatment goals of the primary service coordinator as mandated by WHS.

Provide other medically necessary services to non-TCM clients as determined necessary by the adult team staff.

Other duties as assigned.

Social Service Worker License preferred

Certified Case Manager within 1 month of hire date and renew every 3 years

Valid Utah Driver's License

Defensive Driving and clearance to operate WHS vehicles.

Must be willing to complete and criminal background check and pre-employment drug screening.
