Location: New York, NY
Compensation: $65,000 - $70,000/year
Schedule: 8 AM to 5 PM, Monday to Friday

Responsibilities as the Certified Health Professional:
- Clinical Assessments: Conduct clinical assessments and reassessments using approved questionnaires and techniques.
- Documentation Review: Review medical and health-related documents, including prior documentation, to support assessment outcomes.
- Functional Capacity: Identify customer functional limitations and make functional capacity outcome determinations, coordinating with other clinicians as necessary.
- Collaboration: Work with Engagement Specialists and service track staff to ensure comprehensive service planning and follow-up activities.
- Reporting: Compile and prepare clinical assessment reports, ensuring compliance with HRA policies and procedures.
- Licensing: Must be a Licensed or Certified Health Professional (LMSW, LCSW, LMHC).
- Experience: 2 years of clinical assessment experience, including biopsychosocial assessments and SSA disability evaluations preferred.
- Skills: Strong organizational skills, attention to detail, and exceptional customer service and engagement skills.
- Technical Proficiency: Ability to use electronic systems for documentation and reporting.
