Key Responsibilities:
- Personal Care Assistance: Provide assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting, while respecting the patient's dignity, privacy, and cultural preferences.
- Medication Support: Administer prescribed medications as directed by the hospice nurse or physician, ensuring accuracy and proper documentation in accordance with hospice care protocols.
- Mobility and Ambulation: Help patients with mobility challenges to move around safely, utilizing proper techniques and mobility aids as needed.
- Emotional Support: Offer companionship and emotional support to patients, actively listening to their concerns, fears, and wishes, and providing a caring presence during difficult times.
- Nutrition Assistance: Assist with meal preparation, feeding, and monitoring patients' nutritional intake to ensure their dietary needs are met.
- Communication: Maintain open and effective communication with the hospice care team, including nurses, social workers, chaplains, and other healthcare professionals, to provide comprehensive care and address any changes in the patient's condition.
- Hygiene and Comfort: Ensure patients' comfort by regularly changing bedding, maintaining a clean and organized environment, and addressing any hygiene-related needs.
- Documentation: Accurately document patient care activities, vital signs, medications administered, and any observations in the patient's medical record, in compliance with hospice care regulations.
- Family Support: Offer guidance and emotional support to the patient's family members, helping them understand the caregiving process and providing resources for coping with the end-of-life journey.
- Cultural Sensitivity: Respect and accommodate the cultural, religious, and personal preferences of the patient and their family, creating an inclusive and supportive care environment.
- End-of-Life Care: Participate in discussions and activities related to end-of-life care planning, respecting the patient's wishes and providing comfort measures to ensure a peaceful transition.
Qualifications and Requirements:
- Certification: Must hold a valid Certified Home Health Aide (CHHA) certification from a state-approved program.
- Experience: Prior experience in hospice care, palliative care, or caregiving is preferred.
- Compassion: Demonstrated empathy, compassion, and sensitivity when working with patients and their families during challenging times.
- Communication Skills: Excellent verbal and written communication skills to effectively interact with patients, families, and the care team.
- Reliability: Dependable and punctual, with the ability to maintain a flexible schedule based on patient and family needs.
- Physical Stamina: Ability to perform physical tasks such as lifting, transferring, and assisting with mobility, as required by the patient's condition.
- Ethical Standards: Strong ethical values and commitment to maintaining patient confidentiality and upholding professional standards.
- Cultural Competence: Awareness and respect for cultural diversity, including the ability to provide culturally sensitive care.
- Team Player: Willingness to collaborate with a multidisciplinary care team and actively contribute to a patient-centered care approach.
- CPR Certification: Current Basic Life Support (BLS) and Cardiopulmonary Resuscitation (CPR) certification is often required.
