We are a growing company and have multiple roles open for a skilled Certified Nursing Assistant, CNA. You will provide outstanding patient care and physical support for patients and residents on a daily basis. To do well in this role you should have your Pennsylvania state-certified nursing assistant certification.

QUALIFICATIONS:
- Currently certified in Pennsylvania as a Certified Nursing Assistant.
- Ability to read, write and comprehend English.
- Long-term care experience preferred.
POSITION SUMMARY:
Under the supervision of a licensed RN or LPN performs nursing services based on the facility’s policies and procedures as regulated by the State of NJ, DOH and Federal regulations. Provides assistance to residents with daily routines and other resident activities as needed.
PART I: JOB RESPONSIBILITIES AND STANDARDS
- Provides personal care to residents/guests (bathing, cleaning finger nails, shaving and perineal care, etc.) and assures that resident/guest dresses appropriately.
- Follows infection control procedures (i.e. hand washing/sorting of linens)
- Performs routine /special skin care as indicated. Reports skin alterations immediately to nurse.
- Provides privacy for residents and adheres to HIPAA standards.
- Inquires about residents/guests comfort/discomfort level on a scale of 0-10.
- Provides restorative care as indicated.
- Performs routine catheter care
- Provides care to residents/guests with tubes and special equipment.
- Transports resident/guest to and from activities.
- Answers all call bell and places them in reach of resident/guest.
- Fills water pitchers with fresh ice water unless contraindicated.
- Offers snacks to all residents/guests and documents appropriately.
- Monitors residents during smoking periods when assigned.
- Demonstrates ability to deescalate resident through verbal strategies, redirections and crisis management and report to nurse immediately.
- Assist with meal pass and feeding in a timely manner.
- Follows Fall Prevention Protocol.
- Collects lab specimens.
- Reports condition changes/behavioral issues to nurse immediately.
- Follows elopement risk protocol.
- Maintains cleanliness and organization of resident/guest rooms including bed making.
- Accompanies resident/guests during outside medical consultations and out of building activities.
- Follows safe-handling procedures for personal safety and resident safety at all times.
- Accurately reflects resident needs on ADL Tracker.
- Other duties as assigned.
PART II. DOCUMENTATION
- DOCUMENTS THE FOLLOWING:
- 1. Intake and output/hydration.
- 2. Food intake/calorie count/snack.
- 3. Weights.
- 4. Bowel movements.
- 5. Use of assistive devices.
- 6. Bath/shower.
- 7. Bowel/bladder training
- 8. ADL tracker.
- 9. Observation rounding form (hourly, 30 mins, etc.)
- 10. Individual statement form post incident
- 11. Employee Incident/Accident form to Employee Health, Dept/Supervisor by end of shift.

PART IV: FACILITY-WIDE RESPONSIBILITIES & STANDARDS
EMPLOYEE IS REQUIRED TO:
1. Speaks English in patient care areas.
2. Attends mandatory and scheduled In-services/meetings (12 hrs per year).
3. Arrives to work on time as scheduled
4. Demonstrates ability to work with others cooperatively.
5. Demonstrates flexibility in assignment to meet resident/family needs.
7. Adheres to departmental dress code including ID badge.
8. Responds to following emergency codes: Grey, Yellow, Action Plan, Orange, Red, Silver, E, Blue, Triage, Clear
9. Able to identify and report types of abuse (verbal, physical, sexual, financial, emotional) immediately.#IND123
