NURSING ASSISTANT
PURPOSE:
The purpose of this position is to provide direct care to residents, under the supervision of a licensed nurse, in accordance with the facility policies and procedures and report resident needs and concerns to a licensed nurse.
EDUCATION /KNOWLEDGE:
Must be a graduate of an approved Nursing Assistant Training Program, or be eligible to enter such a program, graduate and pass a state approved competency test (written & skills) within 120 days from the date of employment.
Prior experience as a care provider is preferred.
Must possess the ability to make independent decisions when circumstances warrant.
Must possess the ability to deal effectively with personnel, residents, family members, visitors, government agencies and the general public under all conditions.
Must be willing to follow directions.
Must be dependable in attendance and available by telephone and/or meggsage phone number on reasonable notice.
DELEGATION OF AUTHORITY:
Authority is delegated to the individual in this position to:
1) provide care to residents following facility policies and procedures 2) report residents' needs and concerns to a licensed nurse
DUTIES AND RESPONSIBILITIES:
1. Follow written and oral directions
2. Follow established performance standards and perform duties according to nursing
service policies and procedures.
3. Assist new employees in following established facility policies and procedures,
4. Identify resident problems and concerns and report them immediately to a charge
nurse or a licensed nurse.
5. Complete assignments timely, completely and accurately.
6. Conduct resident rounds as assigned.
7. Initiate corrective action as necessary and/or seek assistance of a licensed nurse.
8. Identify safety hazards and emergency situations, and initiate corrective action.
9. Attend all in-service classes as assigned and complete assignments.
10. Assume personal responsibility for following facility procedures related to control of
equipment and supplies within the unit.
11. Document incidents and/or unusual problems according to established facility
procedures.
12. Participate in interdisciplinary team and other facility meetings as assigned.
13. Participate in the development of an individualized plan of care of assigned resident.
14. Review care plans and perform nursing care as outlined.
15. Document in the nursing assistant notes the care and treatment provided to the
resident and the resident's response or lacke of response to care provided.
16. Report changes in residents' condition to Charge Nurse or Nurse Supervisor.
17. Follow facility procedures for admission, discharge and transfer of residents.
18. Complete documentation accurately, timely and following facility policies and
procedures.
19. Listen to resident and family complaints and report problems to Charge Nurse.
20. Assure residents have call lights at hand and answer call lights promptly.
21. Assure residents are given adequate assistance with meals, nourishment and
transportation to meal service area.
22. Consistently work coopertively with residents, charge nurses, treatment nurses,
restorative nurse, other nursing assistants, physicians, families, consultant personnel and ancillary service providers.
23. Observe all facility safety policies and procedures.
24. Come to work in a clean, neat uniform and consistenly present an appropriate
professional appearance.
25. Come to work as scheduled and consistently demonstrate dependability and
punctuality.
26. Assume accountability for data contained in the employee's handbook.
27. Perform other related duties as directed by his/her supervisor.
28. Perform all duties assigned in an effective, timely and professional manner.
29. Observe infection control procedures.
30. Follow Residents' Rights policies at all times.
31. Be responsible for the safety of residents under his/her supervision.
32. Observe all facility policies and procedures. 33. Accept assigned duties in a cooperative manner.
