About the Job
Position Summary:
The CMC Home Hospice C.N.A. is to provide supportive services to patients and/or residents whose health status is unstable. They provide routine activities or daily living, nursing care, and emotional support in accordance with the patients’ needs, the comprehensive plan of care, and as directed by the supervisor. Responsibilities include provision of temporary relief for the primary caregiver. The C.N.A. is liable for carrying-out the duties assigned to increase the level of comfort and to maintain personal hygiene and a safe, healthy environment for the patient and/or resident.
Job Duties:
- Assist patient with all activities of daily living as well as ambulation, turning and repositioning, and promoting comfort.
- Assist patient with dental and mouth care, bathing, personal hygiene, dressing, bowel, and bladder functions.
- Provide patient with help walking, exercising, and moving in and out of bed. Assist with lifting, turning, moving, positioning, and transporting of residents.
- Manage catheter care and/or incontinence care.
- Use of proper adaptive equipment as outlined by the plan of care or facility’s policies.
- Observe skin and report any impaired skin integrity, or injuries of known and unknown origin.
- Perform incidental household services, make beds, other light housekeeping.
- Ability to handle death and stresses of residents’ illnesses.
- Knowledge of patients’ specific diet and conditions, and customary meal routines.
- Knowledge of patients’ likes and dislikes.
- Obtain meal consumption and other measurements as required and accurately record information in a timely manner.
- Report bruises, skin tears, falls, and all incidents and/or accidents, changes in condition and safety issues to the nurse in a timely manner.
- Accurately and timely report occupational exposures to blood, bodily fluids, infectious materials, and hazardous chemicals.
- Communicates and coordinates daily direct patient care concerns to the Case Manage.
- Knowledge of the types of abuses and when and how to report abuse.
- Assist in orientation of new employees.
- Must be available during normal work hours (unless previously approved by direct supervisor). Additional hours may be required to complete normal business functions and/or projects.
- Assure appropriate and timely electronic documentation.
- Must be able to drive to patient homes, facilities, and healthcare providers locations throughout shift.
- Perform other duties as requested or required, in the sole discretion of Embrace.
Qualifications
Education:
- High school diploma required.
Experience:
- Previous experience working as CNA in hospice and/or related healthcare setting preferred.
Licensure/Certification/Registration:
- Current South Carolina State Nursing Assistant Certification (CNA) in good standing required.
- Current South Carolina Driver’s License required.
Job Expectations:
- Must be able to work flexible hours and travel between offices, facilities, and other places.
- Must be a licensed driver with automobile that is insured in accordance with state and/or organizational requirements and is in good working order.
- Adhere to dress code; appearance is neat and clean.
- Report to work on time and as scheduled, complete work within designated time.
- Represent the organization in a positive and professional manner.
- Communicate the mission, ethics, and goals of the organization, as well as the mission statement.

- Must maintain high ethical standards.
