CLERK, MEDICAL RECORDS BIRTH - Pomona Valley Hospital Medical Center

Position Description: The Birth Certificate Clerk serves as Birth Certificate and Paternity Opportunity Program liaison for the parent. They are responsible for obtaining and verifying for completeness and accuracy of the necessary data elements for electronic completion and transmission of the birth certificate, as required by state law in a timely manner. Responsible for obtaining signature(s) from parent(s) for birth certificate and other state or county specified program(s) (e.g. Birth Certificate, Paternity Opportunity Program, Social Security). Completes and reviews paternity paperwork with parent(s) as needed and submits in timely manner. Maintains confidentiality with all protected health information (PHI) and HIPAA related information. Associate lives the PVHMC values and follows the appropriate behaviors in performing all position accountabilities. May perform other duties as assigned.

Job Specifications: [Required] High School education or equivalent. One year at an acute-care facility. Computer skills essential; basic keyboard, Microsoft Word, Excel essential. Communicates clearly and concisely both orally and written. Ability to work in high volume/fast paced area efficiently with keen ability to multi-task. Self-motivated, needing little or no supervision.

[Preferred] CERTIFICATION: Registered Health Information Technician (RHIT) or Registered Health Information Administrator (RHIA). Bilingual (English-Spanish or English-Mandarin).

Salary range: $18.55 - $26.10
