Comfort Keepers of Anoka and White Bear Lake has a great opportunity for a self-driven, dynamic, and empathetic Client Care Coordinator to join our team. Care CoordinatorQualifications:
Minimum 2 years related experience and/or training.
Experience in senior care or other service industry highly preferred.
Experience in hospital or transitional care settings is preferred.
Must possess and demonstrate excellent communication skills as well as a positive, professional business image.
College degree in health care, marketing management, or a related field.
Requires proficiency in Microsoft Office applications. Will need to master additional software including scheduling systems, calendars, and CRM software.
Responsibilities of the Care Coordinator:
Performs in-home consultations and assessments to establish services for potential clients. Implements contracts and agreements to ensure sales goals are met.
Participates in intake call handling with the sales team to discuss Comfort Keepers services and schedule in-home consultations.
Participate in community activities including but not limited to industry-related committees, boards, community projects, fundraising, etc. to represent Comfort Keepers, initiate and maintain referral relationships as outlined in the business plan.
Performs new networking/sales calls to assess relationship possibilities to referral sources. Target accounts include, but are not limited to: Assisted Living Facilities, Nursing Homes, Skilled Nursing Facilities, Senior Housing, Transitional Care Units, Rehabilitation Services, Hospitals, Home Health Agencies, Hospice providers, clinics, etc.
Performs ongoing care coordination activities including but not limited to reassessments, client advocacy, and spot checks.
Participates in community education presentations and works with appropriate team member(s) to promote Comfort Keepers’s expertise and brand awareness.
Assists in the development and maintenance of prospects, clients, care providers, and referral sources database for relationship management.
Participates in on-call rotation for sales, as assigned.
Other duties as assigned.
