**Job Summary:**
The Client Support Coordinator will provide habilitative assistance and care to clients in various Agency programs, including community and supported living arrangements, family support services, and day/vocational programs. Responsibilities include direct daily care, teaching, and supervision of clients, as well as record keeping. Taking residents on Dr appointments, and doing activities as well.
**Responsibilities:**
- Supervise clients and ensure knowledge of their status and location at all times.
- Assist in the preparation and implementation of each client's Individualized Plan of Service or Individual Educational Plan.
- Teach, supervise, and assist clients with performing adult daily living activities.
- Monitor client progress on goals and protect client rights by reporting incidents of abuse, neglect, and mistreatment.
- Implement behavioral plans and approved/emergency restraints and procedures when necessary.
- Implement therapist recommendations and programs.
- Develop and implement recreation and community participation activities.
- Maintain accurate and timely record keeping of services provided and goals identified in each client's plan.
- Collaborate with Clinical Directors, Consultants, families, schools, and other agencies.
- Administer medications and provide routine health care and first aid to clients.
- Transport clients to and from designated activities and programs.
- Conduct and participate in emergency drills as required.
- Ensure accurate record keeping of petty cash accounts and individual client cash on hand accounts.
- Perform other related duties as requested.
**Qualifications:**
- High School Diploma or equivalent.
- Minimum of one year of experience working with individuals with developmental and/or behavior challenges preferred, or equivalent combination of education and experience. Willing to train right candidate.
**Requirements:**
- Successful completion of Medication Certification Program.
- Successful completion of PMT, CPR, and First Aid training.
- Valid CT driver's license with a satisfactory driving record.
**Physical Demands:**
- Ability to lift and/or move up to 75 pounds and exert over 100 lbs. of force when necessary.
- Ability to stand, walk, sit, use hands to handle objects, tools, or controls, and reach with hands and arms.
- Close vision and the ability to adjust focus required.
**Language Skills:**
- Ability to read and interpret English documents.
- Ability to write routine reports and correspondence in English.
**Mathematical Skills:**
- Ability to add, subtract, multiply, and divide using whole numbers, fractions, and decimals.
**Reasoning Ability:**
- Ability to apply common sense understanding to carry out detailed instructions.
- Ability to deal with problems involving several concrete variables.
**Work Environment:**
The work environment is moderate in noise level and may be exposed to changing weather conditions while traveling by automobile.
**Location:**
This position is for 30 hours per week, primarily in the evenings, located at Charles Street in Watertown, CT.
