**Job Summary:**
Family Options, Inc. is seeking a Client Support Coordinator to provide habilitative assistance and care to clients in various Agency programs in the Watertown and Naugatuck area. The Client Support Coordinator will be responsible for direct daily care, teaching, and supervising of clients, including record-keeping.
**Essential Duties and Responsibilities:**
- Direct supervision of clients, ensuring awareness of their status and location at all times.
- Assist in the development and implementation of Individualized Plans of Service for clients.
- Teach, supervise, and assist clients in daily living activities.
- Monitor client progress on goals and protect client rights.
- Complete incident reports and medication error reports as necessary.
- Implement behavioral plans and therapy recommendations.
- Develop and implement recreational and community participation activities.
- Collaborate with Clinical Director, Consultants, families, and other agencies.
- Maintain accurate record-keeping of services provided to clients and goals identified.
- Attend required meetings, in-services, and trainings.
- Administer medications and provide routine healthcare to clients.
- Ensure cleanliness and organization of program sites and equipment.
- Transport clients to designated activities and programs.
- Conduct and participate in emergency drills as needed.
**Teamwork:**
- Attend staff meetings, in-services, and trainings.
- Collaborate with supervisory staff, Agency personnel, and external partners.
- Communicate effectively and respectfully with all stakeholders.
**Qualifications:**
- Successful completion of Medication Certification Program.
- Successful completion of PMT, CPR, and First Aid training.
- Valid CT driver's license with a satisfactory driving record.
**Education and Experience:**
- High School Diploma or equivalent.
- Minimum of two years of experience working with individuals facing developmental or behavioral challenges, or equivalent combination of education and experience.
**Physical Demands:**
- Ability to perform essential job functions including lifting and moving up to 75 pounds.
- Specific vision abilities required.
- Ability to handle physical restraint procedures.
**Language Skills:**
- Ability to read, write, and interpret English documents.
- Proficiency in writing reports and correspondence in English.
**Mathematical Skills:**
- Proficiency in basic mathematical operations.
**Reasoning Ability:**
- Ability to apply common sense and problem-solving skills.
**Work Environment:**
- Employee may encounter a moderate noise level.
- Travel by automobile and exposure to changing weather conditions may be required.
