About the Job
Seeking an enthusiatic RMA to join our clinic care team as a care coordinator. This position functions as an intregral member of our team providing highest quality of care to our patients and support to our staff. A strong analytical background or interest along with attention to detail and organization is vital to this success of this role. Responsibilities include monitoring reports and gaps in care, chart review, patient education, developing workflows to complete measures, understanding quality measure benchmarks, assisting with patient education, etc. Excellent interpersonal communication skills with a high degree of professionalism in dealing with staff at all levels of the company.

Responsibilities:
- Promotes patient self-management and empowers patient/ families to achieve maximum levels of wellness and independence
- Responsible for working with patient and patient's care team to coordinate change readiness, needs assessment and develop and individualized treatment care plan
- Collaborates with the patients, physician, and other care teams’ members in assessing the patient's progress toward individual health care goals
- Assesses barriers when patient has not met treatment/ goals, is not following treatment plan of care, or has not kept important appointments
- Uses data analytics and reporting for monitoring insurance quality program measures.
- Participates in monthly meetings to gain a better understanding of quality measure benchmarks and standings.
- Performing chart reviews to extract data for necessary reporting
- Ability to develop, prioritize, and accomplish goals
- 2 years related experience and/or training
- Prior Family Medicine experience preferred, but not required
