Division/Program Description
Felton’s Senior Division specializes in a full continuum of mental health services for older adults in outpatient clinics, the community, and supportive housing sites, as well as programs in employment training, economic security, case management, advocacy, and social connection and inclusion. Felton’s Clinical Housing Support Program provides behavioral health and other onsite support services to assist tenants at the Drs. Julian & Raye Richardson and Rene Cazenave Apartments to maintain housing stability and improve access to resources. All tenants were formerly homeless adults with co-occurring mental health, substance abuse, and medical problems, and limited experience living independently. Support services include individual and group behavioral health counseling and case management, psychiatry, primary care nursing case management and medication monitoring, referral to and coordination with primary medical care, substance abuse and psychiatric treatment, benefit counseling and client advocacy, meal programs, health education, community building, tenant organizing, and all other case management functions.
Job Description
Under the supervision of the Program Manager, this position participates in a multidisciplinary team serving tenants of Rene Cazenave and Richardson Apartments many of whom have severe mental illness and co-occurring disorders, providing comprehensive and integrated treatment services, including mental health and substance abuse treatment, case management, and other service connections. The Clinical Housing Support clinical case manager evaluates needs of clients and develops treatment plans, often along with the onsite medication support team.
Specific Responsibilities
- Works closely with an interdisciplinary team of other clinical case managers, registered nurses, psychiatric nurse practitioners, psychiatrists, and community specialists (peer counselors) to provide onsite support to residents in the RCA and Richardson apartments.
- Creates a welcoming community milieu within the apartment building by supporting meal programs, community building events and tenant organizing.
- Collaborates with property management to support tenants to maintain stable independent housing.
- Participates in Officer of the Day rotation to be point person to manage crises, drop ins and wellness checks.
- Performs intakes and comprehensive psychiatric assessments for adults and older adults with psychiatric and dual diagnoses, psychiatric interviewing and DSM V-TR/WHODAS measures of both mental health symptoms and functional impairments
- Provides crisis intervention and management to prevent eviction and hospitalization, and if necessary, facilitates involuntary hospitalization.
- Provides individual and group therapy to stabilize clients and prevent psychiatric de-compensation.
- Provides case management services, including evaluations, service plan development, assisting clients in accessing housing, financial, medical, and other needed services. Coordinates with other service providers.
- Maintains thorough clinical records after each client contact or service, in accordance with mandated standards of San Francisco Community Mental Health Services and funders including Medi-Cal/Medicare
- Participates in team meetings and case conferences through presentation of cases, discussion of clinical issues and exchange of information and ideas.
- Ongoing training by participating in Felton’s training opportunities, including developing specializations in evidence-informed treatments and practices.
- Other duties and responsibilities that may be assigned from time to time.
Minimum Qualification
- Master’s degree or higher in Social Work or other Professional Counseling Programs, and registered with active license with BBS, including intern level ASWs, AMFTs, and APCCs
- Experience in case management and mental health treatment for at-risk adults and/or older adults, including co-occurring disorders (master’s level internship experience considered) preferred.
- Moderate to comprehensive understanding of Specialty Mental Health Services under the Department of Health Care services definitions in addition to skills in assessment, treatment planning, and mental health services pertaining to individuals living with serious mental illness.
- Excellent communication skills both in written and verbal formats
- Knowledgeable about and consultation oriented towards client risks for suicidality/violence, abuse reporting, dual role conflicts, and other ethical/legal obligations as they arise.
- Solution focused care planning and conceptualization skills within multi-disciplinary team and with external agency partners and individuals
- Strong recovery model approach to care and ability to form strong therapeutic relationships with clients from diverse backgrounds and who often are underserved by traditional care models.
- Ability to effectively diagnose and make clinical rationales for psychiatric care utilizing the DSM 5-TR and other measures including social determinants of health and functional impairment screenings where appropriate.
- Ability to work well under pressure while meeting multiple different types of deadlines.
- Computer skills proficiency in applications for electronic health care records, Zoom, Box, Microsoft/Outlook applications, and other technologies as needed for the role.
- Ability to be a team player with a welcoming attitude.

Salary Range: $93,000 to $105,000 Annually (DOE)
