Responsibilities:Supervises the daily operations of Pharmacy Clinical Services for the Hospital, including Pharmacy & Therapeutics Committee and Formulary maintenance. Coordinates with the Director in development and implementation of all clinical pharmacy activities. Supervises Pharmacy Specialists and Pharmacy Residents. Provides leadership, motivation, and educational opportunities to the pharmacy department. Provides consultative services and direct patient care defined as “the direct observation and evaluation of the patient and his/her medication-related needs; the initiation, modification, or discontinuation of patient-specific pharmacotherapy; and the ongoing pharmacotherapeutic monitoring and follow—up of patients in collaboration with other health professionals. The scope of patient care provided may include: General Medicine/Surgical, Intensive Care Services, Obstetrics & Gynecology, Pediatrics, Emergency Care, Oncology, Peri-operative, Geriatrics and Outpatient Services.
Qualifications:EDUCATION, EXPERIENCE, TRAINING
1. Current state Registered Pharmacist License.
2. B.S. degree in Pharmacy required; Doctor of Pharmacy degree preferred.
3. 3 years of previous hospital pharmacy experience required.
4. Strong verbal and written communication skills required.
5. Experience with hospital pharmacy computer systems, spreadsheets, word processing programs, and other databases required.
6. Completion of postgraduate clinical training (PGY-1) or > 3 years of qualifying experience required.
7. Postgraduate clinical training PGY-2 preferred.
8. Board certification in a Board of Pharmacy Specialties clinical specialty required, or must be obtained within 6 months of hire.
