JOB SUMMARY: Liaise between community relations, discharge planners, and Physicians. Must support the Marketing efforts at the community. Performance of clinical assessments for potential and current Westminster Services health center residents. Negotiation for residents who are clients of managed care organizations. Maximize cost effectiveness across the entire continuum of care; improve customer satisfaction and enhance quality of care.
ESSENTIAL JOB FUNCTIONS:
Comply with all applicable rules, policies, standards, and guidelines related to employment with Westminster Services and its communities.
- Work as a community liaison to develop and maintain relations with physicians and hospitals on an ongoing basis, insurance-based case managers, or marketing personnel on all sub-acute patients. including for example: providing them with Medicare updates and changes in facility procedures.
- Ensure that the budgeted census does not decrease below the established minimum.
- Periodically assess services provided by the facility; compare these to the needs of the community. Assess how the current services compare to those offered by other comparable facilities in the area.
- Assist in the evaluation of all potential residents to ensure appropriate placement.
- Ensure proper federal, state and facility mandated documentation is completed. Ensure all necessary Medicare and/or insurance information if available and confirmed prior to admission.
- Works with residents, families and other staff as needed to ensure that residents are evaluated for the appropriate level of care.
- Educate residents and families about the extent of Medicare and Medicaid coverage, including length of stay, amount of coverage, and co-insurance days.
- Address quality measures issues for potential admissions and returning residents.
ESSENTIAL QUALIFICATIONS:
Education/Licensure: Prefer a licensed RN or LPN in the state of Florida; or possess a degree in a related healthcare field. Must be CPR certified.
Experience: Healthcare Business Development experience is a plus. Two years (three years, if LPN) in long term care and assisted living, including working knowledge of case management/ clinical work, Medicare /Managed care and Managed Medicaid protocols. One year in post-acute rehabilitation. Case management purpose and process knowledge required. Demonstrated clinical assessment skills required.
Physical and Environmental Requirements: Grasping, Kneeling, Lifting up to 40 lbs, Pushing, Reaching, Stooping, and Driving.
