This full-time role involves overseeing and coordinating the interdisciplinary team's plan of care, ensuring effective patient care, and supervising Nurse Care Managers.
Location: Bronx, NY
Compensation: $130,000 - $140,000/year
Schedule: 7:00 AM to 3:00 PM, Monday to Friday

Responsibilities as the Clinical Manager, CHAA RN:
- Interdisciplinary Collaboration: Collaborate with the interdisciplinary team to ensure necessary, appropriate, and effective care for patients while supervising Nurse Care Managers.
- Orientation and Education: Develop and implement orientation programs for field staff and provide ongoing in-service education.
- Compliance and Quality Improvement: Participate in compliance and quality improvement efforts to enhance patient care.
- Operational Oversight: Oversee day-to-day operations in the absence of the Director of Patient Services, providing supervision to the clinical care team.
- Team Supervision: Provide oversight and supervision to Nurse Care Managers, Nutritionists, Patient Services Liaisons, Paraprofessional Scheduling Supervisors, and Admissions Liaisons.
- License and Education: Current NY State RN License with a BSN or degree in Health & Human Services; Masters degree preferred.
- Experience: Minimum 5 years in patient assessment and home care planning; 2 years in a supervisory role; PRI assessor skills preferred.
- Technical Skills: Proficient in MS Office.
- Communication Skills: Strong verbal and written communication abilities; effective public speaking and presentation skills.
- Mathematical Skills: Competence in statistical and mathematical concepts.
