About the Job
The Clinical Navigator will work to increase access to Lung Cancer Screening and smoking cessation services for patient who formerly or currently smoke. This person is responsible for supporting the goals of the program and applying empowerment-based prevention and intervention strategies to improve community health specifically related to lung cancer screening, smoking cessation services, making referrals to community-based resources, addressing barriers to improving well-being, offering expertise regarding psycho-social issues affecting patient care, and educating patients regarding illness and treatment options. This position will be required to travel to other LSCC facilities as needed. The Clinical Navigator will provide, facilitate, and track successful referral to resources. The Clinical Navigator will collaborate with employees at all levels within the organization and must be able to maintain professional working relationships and provide excellent customer service.
Responsibilities:- Apply empowerment-based prevention and intervention strategies to improve community health and facilitate smoking cessation education, low dose CT (LDCT) screening, follow-up, and treatment.
- Provide training for providers and staff on lung cancer screening criteria and techniques for increasing referrals to intensive smoking cessation services.
- Provide intensive smoking cessation services.
- Facilitate shared decision making about lung cancer screening. Ensure the timely receipt of screening and timely return of results.
- Help patients requiring additional testing or treatment through patient navigation
- Evaluate patients for additional risk factors, including cancer related one, and provide referrals to meet those needs.
- Assist with data management related to lung cancer screening and smoking cessation services, including tracking of completed tests and management of testing databases.
- Address personal and environmental issues with the patient to maximize emotional, social, and physical wellbeing and the appropriate use of healthcare resources
- Serves as liaison by collaborating with clinic providers, support staff, and outside organizations to assess patient’s needs and provide connection to comprehensive healthcare
- Assist individuals and families by providing case management, care management and crisis intervention limited to scope of practice
- Assist clients and/or families, both in-person and virtually in identifying and accessing community resources to alleviate social, environmental, and/or economic problems impacting health care needs.
- Coordinates, manages, and tracks all aspects of patient referral process and adheres to appropriate timelines
- Perform other duties and projects as assigned.
REQUIRED EXPERIENCE, EDUCATION, AND LICENSE:
- Bachelor’s Degree in Social Work from accredited college or university AND
- Minimum 2 years of experience in a community-based and/or medical setting with an understanding of behavioral health prevention science
- Basic Life Support (BLS) certification from American Heart Association or American Red Cross
PREFERRED EXPERIENCE, EDUCATION, AND LICENSE:
- Master’s degree in Social Work
- LCSW, LPC or related field (licensure required).
- Experience in alcohol screening, use, abuse and/or treatment.
- Experience with screening, brief intervention, and referral to treatment (SBIRT).
- Experience with motivational interviewing (MI) and smoking cessation services.
- Experience working in behavioral health and/or human services
- Proficiency with Electronic Medical Record (EMR) system, as well as computer and web-based interfaces
- Strong understanding of prevention and preferred experience with the SBIRT models, Motivational Interview and other behavioral health screening tools for youth and adults (e.g., AUDIT, DAST, CRAFT, PHQ-9, GAD –7 etc.).
- Knowledge of outcomes assessment, experience with research and evaluation protocols
- Experience in gathering, using, and presenting data for program evaluation information dissemination to stakeholder and community groups, as directed by supervisor(s)
- Bilingual English/Spanish language skills
