Position Overview:
Care Medical Practice is seeking a dedicated Licensed Practical Nurse (LPN)/ Clinical Nurse Coordinator to join our team. In this role, you will assist with clinical and administrative tasks, including patient assessments, medication administration, and coordination of care. You will also support care transitions and ensure the delivery of quality, value-based care. We are looking for someone with a passion for patient care who can work collaboratively within a multi-disciplinary team to improve health outcomes for underserved communities.
Key Responsibilities:
- Comprehensive Patient Care: Conduct in-home and office-based assessments, including vitals, BMI, and blood pressure. Administer immunizations, medications, and provide wound care, ensuring each patient receives high-quality, personalized care.
- Care Coordination: Take the lead in managing patient transitions, follow up after emergency department visits, and coordinate with providers to ensure continuity of care across practices, helping patients achieve better health outcomes.
- Clinical Support: Prepare and complete prior authorizations for medications and diagnostic testing, and assist with medical chart preparation to ensure all necessary medical clearances are obtained for provider visits.
- Patient Education: Under the direction of the provider or Registered Nurse, educate patients on their health conditions, medications, and post-discharge care, helping them manage chronic illnesses and improve their well-being.
- Log and Inventory Management: Oversee clinical supplies, track expiration dates, and ensure compliance with clinical and legal regulations, contributing to a safe and efficient work environment.
Key Qualifications:
- Education: Associate's degree in Nursing and a current LPN license in New York State.
- Experience: Minimum 1 year of nursing experience, preferably in an urban setting.
- Skills: Strong communication skills (bilingual preferred), ability to manage patient relationships.
- Certification: CPR certification preferred.
- Character: Compassionate, flexible, and a strong team player with excellent interpersonal skills.
