Description:
We are seeking a Clinical Pharmacist to join our Practice Extend Team. This role involves providing peer review support in collaboration with our client's Care Team's Care Coordinator and Clinical Administrative Coordinator to enhance patient health outcomes. The Clinical Pharmacist will offer expertise in medication therapy management for patients associated with Provider/Provider Practices in the Practice Extend Program. 

Responsibilities include supporting transitions of care management, optimizing medication therapy, mitigating adverse drug effects, coordinating follow-up care, improving patient outcomes, and reducing the total cost of care for health plans.
