Job Summary and Qualifications
The Pharmacy Clinical Manager is responsible for designing, implementing and managing the pharmacy clinical programs. This includes formulary management, policy and procedure development, pharmacist clinical practice development, and education across the healthcare spectrum. The Clinical Manager shall also assist in managing the pharmacy when the Pharmacy Director or designee is unavailable.
- You will interpret orders and transcribe or verify them accurately and efficiently into the pharmacy computer system
- You will accurately compound and dispense pharmaceuticals
- You will comply with facility policy and procedures and regulatory requirements
- You will monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose
- You will provide clinical consultation and clarification to practitioners. Suggest appropriate, cost-effective therapeutic alternatives to medical staff as needed.

- You will sustain the hospital formulary, minimizing nonformulary procurements, utilizing therapeutic interchanges and promote rational drug therapy selection.
- You will support participation in daily multidisciplinary and bedside rounds (may need to attend).
- You will identify, design, and analyze drug utilization reviews to promote rational drug therapy and determine action plans based on results.
- You will provide accurate and timely medication information to healthcare staff and patients.
- You will develop, manage, and mentor pharmacy clinical programs such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing, and other programs as determined appropriate based on patient population and literature guidelines.
- You will develop, manage, and mentor an antimicrobial management program (AMP)
- You will document all clinical activities and interventions accurately and completely.
- You will be responsible for mentoring and education as well as ongoing analysis of interventions to ensure accurate and appropriate documentation and provide summary reports.
- You will support and participate in the goals of the organization’s quality improvement, risk management, and patient safety program including but not limited to National Patient Safety Goals, Core Measures, and HCAHPS medication-related initiatives
- You will be responsible for design, analysis, and reporting of adverse drug reactions, medication errors, and pharmacy quality initiatives. Mentor others in their roles for these programs
- You will develop and actively participate in training and mentorship programs for pharmacists, pharmacy students, residents, technicians, nurses and physicians Responsible for the assessment of participants in these programs
- You will develop and document staff competency on an ongoing basis and develop improvement plans when deficiencies in competencies are identified
- You will be the primary preceptor for pharmacy students and/or residents as it relates to clinical pharmacy services. Responsible for designing and managing programs and mentoring students/resident as well as other preceptors.
- You will maintain professional competency sufficient to meet current and future needs of the facility. Develop and manage specialty pharmacy practice programs as related to clinical pharmacy services.
- You will provide basic technical support for automated dispensing machines and other pharmacy computer systems. Effectively and proactively intervene in conflicts between pharmacy and medical staff regarding drug therapy issue resolutions.
- You will train and participate in Disaster Team drills and real-event response as assigned.
- You will train and participate with Code Teams as assigned. Obtain and maintain BLS-ACLS or PALS current certification as appropriate.
What qualifications you will need:
- Bachelor of Science in Pharmacy or PharmD from an accredited college or university; PharmD preferred
- Current Texas Pharmacist License
- Required: Minimum of 1 year as a Pharmacy Clinical Coordinator OR Clinical/Operations Specialist OR one of the following: At least 5 years of hospital pharmacy experience required
- Completion of hospital PGY2/pharmacy specialty residency or fellowship
- Specialty certification (eg BCPS, BCOP, BCCCP)
- Preferred: Prior hospital pharmacy management, ASHP accredited hospital pharmacy residency.
