The Clinical Pharmacy Supervisor provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes for patients. The Clinical Pharmacy Supervisor actively participates in multidisciplinary patient care rounds, medication order evaluation and verification, drug distribution, technician supervision, and clinical services including medication histories, pharmacokinetic dosing, drug information requests, and patient counseling to ensure the best medication therapy outcomes. The Clinical Pharmacy Supervisor has responsibility for the daily provision of clinical pharmacy services subject to review and approval of the Director, Clinical Pharmacy Services. The Clinical Pharmacy Supervisor will participate in development of new programs, services, practices, and facilitate scholarly and professional development activities in their area of expertise. The Clinical Pharmacy Supervisor evaluates the performance of Clinical Pharmacy Specialists and Clinical Pharmacists within the designated service line and provides ongoing feedback. Oversees and participates in educational activities of the pharmacy department and in the development of pharmacy residents and students. If Clinical Pharmacy Supervisor is appointed by pharmacy leadership as a PGY1 or PGY2 Residency Program Director (RPD) they have the additional responsibility of oversight of the residency program.
Responsibilities
Major job responsibilities include: medication order evaluation (to include but not limited to appropriateness and effectiveness) and verification, drug distribution support, technician supervision, clinical intervention, formulary management, and resource for nursing and physician medication related inquiries. They will serve as a subject matter expert collaborating with pharmacy leadership in the development and implementation of clinical pharmacy services and ensure compliance with policies and procedures. If the Clinical Pharmacy Supervisor also serves as RPD they will have the additional responsibility of overseeing the pharmacy residency program.
The following activities are components of the position necessary to support the major job responsibilities:
Deliver comprehensive pharmaceutical care to patientso Work collaboratively with other clinicians and health-care providers to implement and maintain innovative disease management programs and clinical pharmacy serviceso Participates in the development and implementation of clinical pharmacy goals that are in alignment with the organization’s mission, vision and strategic plano Review medication orders for accuracy, appropriateness and efficacyo Document all pharmacy interventions in patient electronic medical record and integrated clinical decision support systemo Monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions
o Provide pharmacokinetic consultations and other dosing recommendations as required o Prevent, detect, monitor, document, and report adverse drug reactions and medications errorso Participate in the drug therapy management of medical emergencies (code blue, code stroke, etc.)o Perform both admission history and discharge medication review and/or counseling in order to facilitate the admission/discharge/transfer of patients o Provide clinical and operational support to pharmacy team members as determined by departmental needo Respond to drug information requests and optimize pharmacotherapy regimens utilizing current evidence-based literature
Promote and demonstrate rational, cost-effective drug therapyo Participate in formulary management by implementing therapeutic interchanges and enforcing the non-formulary ordering processo Assist in the procurement of medication when necessary, including verification of home medication supply, when applicableo Facilitate the implementation of clinical and operational initiativeso Document clinical consultations and cost avoidance as part of the departmental clinical documentation programo Prepares formulary review and drug use evaluations of medications used in the area of clinical expertise for presentation to the Pharmacy and Therapeutics Committee o Assist pharmacy leadership in the determination, communication, and education of alternative therapies during medication shortageso Provides monthly and/or periodic reports to administration containing qualitative and/or qualitative datao Promotes compliance with established policies within the pharmacy department and the Pharmacy & Therapeutics Committeeo Provides guidance to medical staff and other clinical providers on evidence-based medication therapies and protocols in area of expertise
Facilitate and participate in scholarly and professional development activitieso Provide guidance and supervision to certified pharmacy technicianso Precept pharmacy students during IPPE and APPE rotations as necessaryo Precept PGY1 and PGY2 residents o Participate in staff development (preceptor development/continuing education) for all pharmacy learners within area of expertiseo Coordinate research efforts within area of expertiseo Active participation in local, regional, or national organizations in area of expertise encouragedo Collaborate with pharmacy leadership in developing education, training, and competency programso Design and deliver didactic lectures to Colleges of Pharmacy, Nursing and/or Medicine
Execute hospital and pharmacy policies and procedures o Collaborate with pharmacy leadership in development and review of departmental policies and procedureso Supervises all aspects of ongoing pharmaceutical care to include staffing levels, as well as, the performance of clinically based services such as patient rounds, clinical assessments, therapeutic drug monitoring, pharmacokinetic dosing, nutritional support, antimicrobial surveillance, etco Function as a role model and as an educational resource for pharmacy staffo Recruitment and orientation of new staff memberso Complete sterile compounding training, including hazardous medications, and demonstrate competency on a frequency defined by the Sterile Compounding leader
o Complete extemporaneous compounding training, including hazardous medications, and demonstrate competency on a frequency defined by pharmacy leadershipo Perform quality assurance activities as defined by pharmacy leadership, including, but not limited to, medication history auditso Cover assigned shifts as determined by departmental needo Facilitate investigational drug studies with assistance of Investigation Drug Service Manager and/or other personnel involved in the drug study

PATIENT POPULATION SERVED
The Pharmacy Department provides pharmaceutical care to neonatal/infant, pediatric, adolescent, adult and geriatric populations.
Qualifications
Education / Accreditation / Licensure (required & preferred):
• Graduated from an accredited College of Pharmacy• Licensed or license eligible as a pharmacist in the Commonwealth of Kentucky• Current board certification by Board of Pharmacy Specialties® within area of expertise if available, otherwise general board certification required• Current Basic Life Support and Advanced Cardiac Life Support certification or must obtain within 90 days from date of hire
Experience (required and preferred):
• Training/Experience:o Post-Graduate Year 2 (PGY-2) Specialty Residency required plus 3 years of experience in specialty area ORo Post-Graduate Year 1 (PGY-1) Pharmacy Residency required plus 5 years of experience in specialty area ORo Ten years of experience in specialty area
