Shift: Shift 1
Work Schedule: 8 Hr (8:00:00 AM - 5:00:00 PM)
Summary:
The Cancer Center Clinical Programs Patient Navigator will be responsible for navigating patients into and through clinical programs including routine cancer care and supportive care. With an emphasis on underserved communities, they will help guide patients through the complexities of care across the cancer care continuum. The navigator will play a special role not only in navigation but also in community outreach and education. This navigator will be held under the Cancer Center Supportive Oncology with a dual reporting structure to the Director of Supportive Oncology and the Cancer Center DEI Officer. This navigator will work closely with the Clinical Trials Office.
Other information:
• Associate or bachelor’s degree
• Preferred 1 year of patient navigation.
• Proficiency in Spanish ideal.
• Excellent interpersonal and communication skills, with the ability to build trust and rapport within the community.
• Preferred one year experience in an electronic medical record/preferably EPIC.
• Ability to function independently and organize.
• Experience working in diverse communities.
• Demonstrated cultural sensitivity, awareness, and understanding of diverse backgrounds, including race, ethnicity, gender, sexual orientation, age, disability, education, and socioeconomic status.
• Passion for promoting health equity.
Responsibilities:
• Provide support and navigation to patients presenting for cancer care who have identified needs impacting engagement, adherence, and access to comprehensive care.
• Work closely with clinical and supportive care teams.
• Daily monitoring of a caseload of patients with navigation needs
• Provide outreach for patients due for follow-up, labs, testing, and questionnaires in relation to their cancer care and supportive care.
• Prepare and deliver routine correspondence to the medical teams when appropriate.
• Monitor compliance with scheduled appointments.
• Document all correspondence in electronic medical records as necessary.
• Document into a database when appropriate.
• Work to improve identified outcomes and the experience of a patient who is receiving cancer care.
• Work with the care team to identify barriers to adherence with care (i.e. transportation, financial concerns, child care) and work with the patient, patient’s family, and team to alleviate barriers through social work and other supportive oncology resources.
• Participate in community outreach and education events.
