Pay: $76.13 - $79.04 per hour, depending on education and years of experience
Job Summary:
The Clinical Pharmacist is an advanced practice clinical pharmacist who specialize in mental health care and has extensive knowledge of medication management. The Clinical Pharmacist will develop a program supporting the Behavioral Health Units and will be responsible for decreasing readmission rates and reducing cost. This position will support our clinical services at both our main campus and our offsite locations. Additional responsibilities include but are not limited to:
- Medication reconciliation
- Error reduction
- Reducing the length of stay
- Medication adherence
- Reducing cost
Inpatient Unit Duties:
- Chart review patients and round with the interdisciplinary team
- Provide recommendations regarding medication therapy
- Be a reliable and knowledgeable resource to the team
Outpatient Clinic Duties:
- Medication Therapy Management (MTM) Physician interventions: Duplicate therapy, drug interactions, adverse drug monitoring
- Patient medication & health education counseling
- Follow-up calls & appointments
- Confirm outpatient prescriptions are filled
- Resolve insurance/prior authorization issues
Requirements/Skills:
- NYS Pharmacist License Graduate of ACPE accredited School of Pharmacy
- Graduate of PGY1 residency or Board Certification
- Must possess current license to practice as registered pharmacist in N.Y.S
- Completion of 1 year Pharmacy internship.
- Must fulfill NY State required continuing education for pharmacist
- Minimum 2 years hospital pharmacy experience preferred
- Knowledge of dosage requirements and precautions for neonatal, pediatric, adolescent, adult, geriatric, acute care, and psychiatric patients
- Expertise in pharmacology, monitoring of drug interactions, therapeutic outcomes, MUE's and Drug Information Service Competency in dispensing pharmaceuticals and preparing IV mixtures
- Ability to bend, stretch, and reach necessary for handling/fingering labels, vials, and apparatus used for compounding medications.
- Verbal ability is needed to interpret and and understand written prescriptions, formularies, and reference texts, as well as for presenting data and information on pharmaceuticals to physicians, staff and public.
