POSITION SUMMARY:
As acting Agency Manager, directs all Home Health Care Clinical Services ensuring comprehensive, competent and coordinated patient care. Develops and implements business and strategic plans ensuring optimal delivery of clinical services, integration of physicians as well as successful financial performance. Manages delivery of service assuring compliance with governmental regulations and established accrediting agency standards. Participates in continuous quality improvement activities and educational experiences in support of departmental philosophy and objectives, as well as, Health System initiatives.
EDUCATION/TRAINING:
Minimum:
Master’s Degree in Nursing, Health Sciences or Business Administration required. If Master’s degree is not in nursing, a BSN is required.

LICENSURE/CERTIFICATION:
Minimum:
- Current RN licensure in the State of Illinois.
- Biannual CPR.
- Valid Illinois driver’s license.
- Proof of liability insurance.

EXPERIENCE:
Minimum:
At least three years of recent community nursing experience and at least three years of management experience.
Preferred:
Experience with OASIS data collection.

Successful completion of annual clinical, age and job specific competencies and skill verification tools required.

An understanding of The Joint Commission and State of Illinois, as well as federal regulations, related to Medicare Part A Home Health strongly preferred.
MAJOR RESPONSIBILITES:
•
Defines and establishes department goals, objectives, strategic and marketing plans and patient care standards. Formulates and communicates department policies that link care between the Health System and the community and integrate the organization’s values, service excellence philosophy and assure Federal, State and Regulatory compliance and accreditation.

Plan, coordinate, manage, and evaluate the activities of a multidisciplinary team to ensure the delivery of high quality home care services to patients.

Be proficient in and understand Medicare Home Health Conditions of Participation and State of Illinois standards for home health services.
•
Supervises and ensures appropriate staffing of the Agency on a twenty-four hour per day basis. Monitors, evaluates and takes appropriate actions to evaluate staff performance and ensure staff competency and compliance with department and hospital rules and regulations and human resources policies.

Monitors department operations and outcomes, productivity and compliance with regulatory requirements evaluating the effectiveness and quality of services. Directs the care that is delivered to each patient in accordance with their individual needs, quality standards and the physician’s plan of care.

Develops and manages department operating and capital budgets analyzing statistical data to forecast performance ensuring an appropriate mix of cost savings and revenue generating procedures.

Monitors and maintains clinical records as required by regulatory and accrediting agency standards.
•
Utilizes technical knowledge and expertise to function as a coach, resource and mentor hiring, disciplining, motivating, developing and educating staff, facilitating problem resolution and maintaining an environment which promotes positive change strategies and individual growth.
