Sign-On Bonus: $15,000 + $1,000 from our agency! 💰
Salary Range: $62,000 - $85,000 (based on education and experience) 💲
Employment Type: Full-Time (Days, 8 AM - 4:30 PM)
About the Role:
Join our Care Management Triad Team! You’ll collaborate with RN Case Managers and Utilization Management to enhance patient care and support discharge planning.
Key Responsibilities:
Address psycho-social barriers to patient care.
Collaborate with healthcare professionals to create effective discharge plans.
Review patient status for appropriate care and payer coverage.
Qualifications:
Must have a Master’s in Social Work (MSW or MSS)
Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) required before start date
Minimum 2 years of experience in acute care
