Summary:
The Clinical Support Manager (RN) works closely with the Senior Director of Operations assisting in ensuring that all aspects of nursing care and services are coordinated and managed according to Hospice of Holland nursing standards.
Responsibilities of the Clinical Support Manager (RN):
- Participates in on-call nursing supervisory rotation on weekends, evenings, and off-hours. Acts as a back-up to other Hospice of Holland Clinical Managers.
- Assists the Senior Director of Operations with the supervision of all Clinical Care staff members and is primarily responsible for the clinical care delivered by HOH on-call nurses and all hospice Aides.
- Assists the clinical services team to achieve the Hospice of Holland strategic plan while continuing to identify opportunities for improvement. Participates in customer service resolution if/as needed.
- Assures team compliance with local, state and federal laws and other regulatory standards in all matters relating to nursing. Participates in Medicare surveys (announced and unannounced) as needed.
- Primarily responsible for the evaluation of Hospice clinical care services for all staff directly under their supervision. Promotes quality and safe delivery of patient care.
- Reviews and evaluates team competencies through a variety of means such as home and facility visits, care conferences, record and documentation review, and remains available during operating hours to assist clinicians as needed.
- Reviews patient's medical diagnosis, prognosis, medications, procedures, and clinical care for their team as appropriate to ensure cost-effectiveness and regulatory compliance. Participates in and helps oversee inter-disciplinary team meetings (IDG).
- Assists clinicians in establishing immediate care needs during off-hours, and primarily responsible for call system and call center activities.
- Assists the Senior Director of Operations in the screening, selection, and orientation of new employees.
- Participates in the development and review of all Hospice of Holland nursing policies and procedures.
- Assures compliance with local, state, and federal laws and other regulatory standards in all matters related to clinical care.
- Assists with staffing and scheduling for clinical care staff under their supervision in a timely manner.
- Ensures clinical nursing documentation in the electronic medical record (EMR) meets Hospice of Holland standards.
- Assists with oversight of the Hospice of Holland clean supply room, durable medical equipment and supplies, medication lock-box, and use of pharmacies associated with Hospice of Holland.
- Helps ensure every person under Hospice of Holland care receives the best care possible.
Qualifications for the Clinical Support Manager (RN):
- Graduate of an accredited school of nursing with a BSN degree, masters preferred.
- Previous experience in hospice/home care setting with two years of clinical nursing experience. One year of supervision is preferred. Proven ability to work within an interdisciplinary setting.
- Registered Nurse with current licensure to practice nursing in the state of Michigan. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
- Possesses excellent leadership skills, able to motivate and build a strong team, and motivate staff to continue to pursue excellence in work standards and inter-work relationships.
- Has excellent nursing skills/judgment and understanding of Hospice philosophy.
- Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures, and governmental regulations.
- Ability to participate in the writing of reports, business correspondence, and procedure manuals.
- Ability to present information in one-on-one and small group settings.
- Ability to define problems, collect data, establish facts, and draw valid conclusions.
- Knowledgeable and demonstrates understanding of accepted professional standards and practice.
- Ability to respond to and manage constant change and multi-task.
