Position Details:

We are seeking a skilled and compassionate Cognitive Wellness Provider who specializes in conducting assessments and creating personalized care plans for individuals experiencing cognitive challenges. As a part of our team, you will play a vital role in improving the quality of life for our clients by accurately assessing their cognitive status and developing tailored care plans to enhance their overall well-being.

Responsibilities:

•  Conduct comprehensive cognitive assessments to evaluate memory, attention, problem-solving skills, and other cognitive functions.
• Analyze assessment results to identify strengths, weaknesses, and areas of improvement for each patient/resident.
• Collaborate with caregivers, and other healthcare professionals when needed and develop individualized care plans tailored to the patient's (resident) specific needs and goals.
• Monitor and track the progress of each patient /resident through regular assessments and adjustments to the care plan as needed.

Requirements:

• Provider (NP license)
• Licensed in at least five of the following states; FL, CT, IA, IN, KS, MA, MI, MO, NE, NH, NV, NY, RI, VT, WA
• Experience working with individuals with cognitive impairments, dementia, or related conditions.
• Strong interpersonal and communication skills to collaborate effectively with clients and multidisciplinary teams.
• Empathy, patience, and a genuine interest in promoting cognitive wellness and independence in others.
• Ability to create detailed and individualized care plans based on assessment findings
• Proficiency in using technology for assessment, documentation, and communication purposes.
• Commitment to ongoing professional development and learning in the field of cognitive wellness.

Salary Range: $48 per patient based on CPT code - 16 pts per day ($200,000 per year)
