What you’ll do as a Community Connector:
Location and Schedule:This position will provide support to people with disabilities in Duluth, MN. Job duties will be performed within the person's home, their community, and in an office environment. Hours are scheduled based on the needs of service recipients but will typically include Monday-Friday day and evening hours with occasional weekends required. Access to a vehicle is required for travel with mileage reimbursement provided.

What you’ll do as a Community Connector:
- Working 1:1 with referred individuals, implement a three-step process of discovery, connection, and sustainability for the purpose of helping the person build meaningful relationships and connections within their community.
- Identify a person's interests, skills, and ideal conditions. Assess and teach skills in an effort to develop a network of opportunities allowing the individual to connect to and flourish in their desired community.
- Develop Individualized Support Plans based on assessment.
- Evaluate and assess progress of community connection goals, identify barriers, and find solutions to problems.
- Participate in events and activities with individuals receiving services to help them integrate into their community.
- Connect with community members, organizations and events to build awareness of the community connection service.

For this position, you will need:
- An associate's degree in human services, communication, or related field or one year of equivalent experience.
- Two years of work experience supporting individuals with disabilities with responsibilities that included planning program activities, coordinating projects, and connecting with community resources.
- To pass a post-offer background check.
- A valid driver’s license, access to a vehicle for travel, and be able to pass an MVR check.
- To be able to genuinely connect well with people with disabilities and community members.
- Strong interpersonal, communication, and networking skills.
