This role involves providing skilled care and routine clinical nursing assessment visits to PACE members and families, primarily in the members home but also in various healthcare settings.
Location: Queens, NY
Compensation: $102,000 - $114,000/year
Schedule: 8:30 AM to 5:00 PM, Monday to Friday

This role involves providing skilled care and routine clinical nursing assessment visits to PACE members and families, primarily in the members home but also in various healthcare settings.

Responsibilities as the Community Health Nurse (RN):
- Care Coordination: Provide skilled nursing care and routine clinical assessments in compliance with applicable laws, regulations, and agency policies.
- Assessment: Conduct comprehensive health care needs assessments, including physiological, psychological, social, and environmental factors.
- Care Planning: Participate in PACE comprehensive care planning meetings, collaborate with the interdisciplinary team (IDT) to develop and revise care plans based on member-specific needs and goals.
- Implementation: Administer medications and treatments as prescribed, apply appropriate nursing procedures, and revise care plans as needed.
- Education: Teach patients and caregivers self-care techniques, medication management, diet, safety, and treatment modalities.
- Health Counseling: Provide health counseling and education to patients, families, and caregivers to promote, maintain, and restore health.
- Eligibility Confirmation: Review and confirm PACE program eligibility based on Nursing Facility Level of Care (NFLOC) score.
- Care Coordination: Perform care coordination across all settings, collaborating with internal and external healthcare professionals to ensure continuity of care.
- Education: Degree from an NLN accredited school of nursing required; BSN preferred.
- Experience: Minimum of 1 year general clinical nursing experience in a certified home health agency, acute care, medical surgical, and/or critical care experience. Experience with a frail or elderly population is preferred.
- License: Current New York State license to practice as a Registered Nurse.
- Drivers License: Current drivers license, required insurance, and car available for work as required.
