Compensation: $105,000 - $118,000/year
Location: Queens, NY (Fieldwork throughout Queens)
Schedule: Monday to Friday, 8:30 AM to 5:00 PM
Responsibilities as the Community Health Nurse:
Become a vital part of our client's team in Queens, NY, as a Community Health Nurse, delivering in-home nursing care to PACE members. Responsibilities as the Community Health Nurse:
- Nursing Care: Deliver skilled nursing services, including assessments, medication administration, and care plan implementation for PACE members.
- Care Coordination: Collaborate with the Care Manager and interdisciplinary team (IDT) to develop and revise individualized care plans, ensuring compliance with state and federal regulations.
- Assessments: Conduct initial and follow-up assessments using the Uniform Assessment System of New York (UAS-NY) to confirm eligibility and address care needs.
- Counseling: Provide health education and counseling to patients, families, and caregivers on self-care, medication management, safety, and treatment plans.
- Documentation: Maintain timely and accurate documentation in the electronic medical record system and meet all regulatory standards.
- Education: Degree from an NLN-accredited nursing school required (BSN preferred).
- Licensure: Active New York State RN license required.
- Experience: 1+ year of clinical nursing in home health, acute, medical-surgical, or critical care, with required home care and field experience.
- Skills: Strong clinical abilities, physical endurance, and mental resilience required.
- Other Requirements: Current driver's license, insurance, and access to a vehicle for work.
