Job Summary/Objective:
- Community Health Worker’s (CHW’s) provide customers at First Step, Summit Pointe’s Psychiatric Urgent Care facility, linkage and connections to health and social services throughout Calhoun County. These social services include, but are not limited to economic, health, educational, social, and housing resources. CHW’s engage customers at First Step to understand services they are receiving while at the facility as well as to support and monitoring care planning for customers following their evaluations at the facility. CHW’s maintain a strong awareness and ability to navigate public welfare programming. CHW’s work closely with Summit Pointe programs including Peer Support Specialists, Recovery Coaches, Clinicians, Technicians, as well as Summit Pointe’s Integrated Care team.
Essential Duties and Functions:
- Assist customers in the development of their post visit plans.
- Conduct ongoing engagement with the customer to support their success and connection to social, behavioral, and physical health resources.
- Maintain record compliance within agency requirements and standards utilizing electronic / hard copy record formats.
- Ensure compliance and participation with competency improvement activities including, but not limited to credentialing, continuing medical education (per licensure), and professional improvement practices.
- Participate in corporate quality improvement and problem-solving activities.
- Maintain confidentiality and privacy within MDCH, state and federal guidelines.
- Utilize knowledge of and act in accordance with CARF, CMS, MDCH, CCBHC, and Managed Care standards.
- Perform other duties as required.
Qualifications:
Educational
- High School Diploma or equivalent.
- Must be eligible to participate in and complete MiCHWA Certification within 1 year of hire.
Basic Qualifications
- Must Reside in Calhoun County, MI.
- Good organizational, verbal/written communication, computer, analytical and problem-solving skills.
- Self-directed and resourceful.
- Two years of experience within a medical setting or behavioral healthcare setting is preferred.
- Specialized training, skills, and abilities: Lived experiences with behavioral health issues are desired.
- One year of computer experience – Word, Excel, and electronic medical record preferred.
Trauma Informed System of Care:
A trauma-informed approach to service delivery emphasizes physical and emotional safety, choice, empowerment, and trustworthiness for customers. The impact of trauma is broad and touches many life domains. Participation is mandatory for activities required for Summit Pointe to become a “Trauma Informed System of Care.”
Physical Requirements:
- Professional office environment that generates noise from computer equipment/peripherals.
- Possible exposure to health risks (violence and infectious diseases).
- Sometimes lifts, carries, or moves objects up to 25 lbs.
- May be required to work variable hours/shifts.
