Responsibilities
As the Community Health Worker within the Case Management Department, you will telephonically perform the initial health screening for newly enrolled members and conveys the purpose and services of the case management programs. This job provides basic health information, collects data and performs clerical duties that support both the RN Case Manager and Social Worker. You will also work with the health care team in support of case management goals, care transitions and health literacy to optimize the member's care. Other responsibilities include:
+ Telephonically performs the initial screening for all newly enrolled DCHP members.
+ Supports members in accessing health and social related services, i.e. finding PCP, assist with scheduling appointments and accessing the value added.
+ Uses electronic resources and registries in collecting, reporting, gathering and documenting data for the case management process.
+ Assists with monitoring preventive health care screenings and open care gaps and conducts outreach to link the members back to their medical home to have the screenings completed.
+ Serves as a point of contact by phone and in person for members receiving case management and social work services and others for follow-up activities.
+ Performs a range of non-clinical case management functions including but not limited to outreach, social support, resource linages, interpretation/translation, and transportation.
+ May act as preceptor for newly hired case management coordinators, assist with quality improvement activities, and participate in special case management projects.
+ Promotes individual professional growth and development by meeting requirements for mandatory skills competency, build leadership skills for yourself and others in the department.
+ Performs delegated department operational duties as assigned.
+ Performs other job related duties as assigned
Requirements
Education:
+ High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
+ Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
Additional Preferences
Additional Preferences:
+ 2+ years of experience in patient care of public health setting OR 2+ years as a Medical Assistant strongly preferred
+ 1+ year of Case Management experience strongly preferred
+ Certified Community Health Worker (CCHW) or Certified Medical Assistant strongly preferred
+ Bilingual in Spanish strongly preferred
