GENERAL DESCRIPTION
The Community Health Worker serves as a liaison between patients and health and social services and the community to facilitate access to services and to improve the quality of service delivery. A Community Health Worker will serve approximately 200 patients and work with Senior Health Express (SHE) to provide a variety of services to the patients.
AREAS OF RESPONSIBILITY

The following is a list of essential functions, which may be subject to change at any time and without notice. Management may assign new duties, reassign existing duties, and/or eliminate function(s).
•Coordination with Care Team: Collaborate closely with Care Team members (healthcare providers, nurses, clinical coordinators, etc.) and other staff members to ensure coordination of patient care and compliance with the care plan. Communicate with providers and staff via phone, email, Teams, Athena or in person.
•Coordination with Health Coaches: Work with Health Coaches to relay information regarding patient issues in regard to meeting the goals of the patient’s care plan.
•Patient Services: Conduct or coordinate the following patient services.
oDeliver medications to patients’ homes and review instructions with them.
oPerform medication reconciliation, comparing the patient’s current medication list with the prescriptionand over-the-counter medications found in their home.
oCoordinate and facilitate telehealth visits between patients and pharmacists, specialists, and primarycare providers.
oCollect and draw laboratory samples from patients for testing.
oSet up and instruct patients on Remote Patient Monitoring (RPM) equipment.
Make recommendations and provide training on medication administration alternatives such as LIVI.
•Documentation: Document appropriately and accurately in electronic medical records.
•Patient Communication: Interact with patients via phone, email, patient portal, telephone, text, in person, etc. to gather necessary information.
•Customer Service: Provide exceptional customer service to patients, exhibiting a compassionate and empathetic attitude. Address patient concerns, inquires, and complaints professionally and promptly, striving to meet patient needs and ensure their satisfaction.
•Administrative Functions: Perform general administrative tasks and support other staff as needed. Utilize the company’s software systems to enhance patient care and staff productivity.
•Collaboration and Coordination: Collaborate with internal and external resources to facilitate and ensureseamless operations.
•Participate in coaching calls.
•Perform other duties as requested or required, in the sole discretion of the Company.
