Position Overview:

POSITION DESCRIPTION
Summary of Responsibilities
This position utilizes a holistic approach, to encourage access to healthcare to meet medical/physical/ psychological health needs and promote the general well-being of referred Geisinger Health Choices Members. The emphasis is to increase awareness of preventative health care measures, educate on the proper use of emergency departments, and the benefits of establishing a relationship with primary doctors. Services are provided in accordance with all existing policies, procedures, and regulations and in accord with the mission and values of the company.
Essential Functions
Engaging with Members in the Community and Coordinate Care
Conduct outreach to identified community members. Assess areas of need of coordinate available community resources for individuals served. As needed, conduct assessments and complete required processes.
Advocate
Collaborate with individuals served and community partners, as needed, to ensure that individuals served have the necessary care and resources. Support individuals so that they have the best possible healthcare in the community.
Educator of Health Needs
Educate individuals served on available resources relating to their healthcare. Provide information and resources as needed to ensure that individuals served have available knowledge relating to their healthcare needs.
Provide Continuity of Care
Meet with assigned individuals and complete the assessment process. Identify areas of need and coordinate support as needed. Throughout the process and involvement, ensure to follow all necessary rules and regulations including, but not limited to, policies relating to privacy and HIPAA.

Education and Experience
Education/Experience: Minimum high school education is required and a minimum of one year of experience in community health or case management is required.

- Empathy and understanding of diverse community needs.
Preferred Qualifications
Bachelor's degree in a related field such as Public Health, Social Work, or Community Health.
Employment Requirements
Automobile and Valid Insurance, Valid Pennsylvania Driver's License and Acceptable Motor Vehicle Record Check, Act 33 and Act 34 Clearances, FBI Fingerprint Clearance, Social Security Card, Immigration and Naturalization (I-9), Drug Screening Clearance
Working Conditions
Prolonged periods working on a computer in an open office environment or mobile, comfortable communicating through virtual technology, daily travel in local county area(s) to various provider and consumer locations, some travel may occur during adverse weather conditions, must be able to lift up to 15 pounds.
