About You
- Must be from and representative of the served community
- Associate’s degree or equivalent in human services, public health, social work or similar field
- At least 1 year of non-clinical case management/care coordination experience in Baltimore and Baltimore County, MD
- Valid driver’s license and auto insurance in candidate’s name as a driver
- Reliable transportation or presence of a transportation plan
- Current CPR/BLS certification
- Comfort working primarily in the community in a diverse, fast-paced environment
- Strong, practiced familiarity with Buffalo-area public assistance programs, social service providers, and community-based organizations
- Experience with electronic medical records preferred
- Experience working as part of a diverse team.
About the Job
The Community Health Worker (CHW) will provide non-clinical member outreach and engagement, screening, and assessment, coordination of non-clinical services and supports, family and psychosocial support and health promotion coaching, and monitoring of member well-being throughout the perinatal period.
Primary Responsibilities
- Complete all required member documentation
- Conduct outreach, engagement, and initial screening/assessment with prospective members
- Coordinate non-clinical services and supports listed in the member’s plan of care.
- Process prospective member referrals and dispatch CHWs to conduct initial screening and enrollment
- Provide family and psychosocial support and health promotion coaching
- Enter case notes and supporting documentation into member case files
- Monitor general well-being of mom, baby, and family throughout the perinatal period
- Support member transitions of care as directed by Program Manager
- Support member participation in program-based health education
- Participate in daily huddles and weekly case conferences
- Maintain lines of referral across SDOH and other social service partners/providers

The working environment and physical requirements of the job include:
This position requires both field and office-based work. The job requires frequent travel to community partners and non-home-based market offices in all types of weather conditions. In-office work is performed indoors in a traditional office setting with conditioned air, artificial light, and an open workspace.
In this position you will need an ability to travel frequently by car and/or public transportation, the ability to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 30 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
