The Community Health Worker (CHW) is responsible for bridging the gap between healthcare providers,
community services providers, and individuals in the community to promote health, reduce disparities,
and improve service delivery. The Community Health Worker is a trained and trusted public health
worker who is respected by the people they serve and applies his/her unique understanding of the
experience, socio-economic needs, language, and/or culture of the communities served.
Responsibilities
• Helping patients and caregivers to access health resources: Works with Public Health Specialists,
Health Educators, Case Managers, Social Workers, Primary Care staff, and community agencies
to link patients to needed preventative health services to prevent illness, improve skills for selfmanagement of chronic conditions, and promote healthy lifestyle behavior change.
• Provides health education to MPG patients regarding the primary care medical home and
available resources such as the Virtual Care Clinic, Walk-in Clinics, and numerous value-based
service offerings available to Millennium patients. Facilitating communication and client
empowerment in health care and social service systems interactions.
• Links individuals to community resources to meet basic needs that influence health (i.e. housing,
food, job placement, financial assistance, literacy assistance).
• Ensuring that health care and social service systems are culturally relevant and responsive to
their service population. Delivering health information using culturally appropriate terms and
concepts. Conducting checks on individuals with specific health conditions. Providing informal
health coaching and support to patients.
• Staffing tables at community events.
• Assisting patients with applications and paperwork.
• Identify barriers to health and advocate for changing service delivery and other strategies to
improve community health.
• Demonstrate excellent guest service to internal team members and patients.
• Perform other related duties as assigned.
Qualifications
• High school diploma. Bachelor's degree in social work or a related field is preferred
• 3+ year of experience as a Community Health Worker or in a similar role, preferred.
• Experience working with people of different cultures and socio-economic backgrounds.
• Understanding of the healthcare system and how to help patients find necessary resources.
• Spend 80% of their time doing outreach, health education, and navigation in community settings
• Having a unique understanding of the served populations and the community.
• Ability to work independently in a fast-paced, cross-functional environment.
Physical Demands
• Sedentary work. Exerting up to 10 pounds of force occasionally and/or negligible amount of
force frequently or constantly to lift, carry, push, pull, or otherwise move objects. Repetitive
motion. Substantial movements (motions) of the wrists, hands, and/or fingers. The worker must
have close visual acuity to perform an activity such as: preparing and analyzing data and figures;
transcribing; viewing a computer terminal; extensive reading. Ability to lift to 15 lbs.
independently not to exceed 50 lbs. without help.
