The University of New Mexico isseeking a Lead Community Health Worker to join the Office for Community HealthAdult Inpatient Social Determinants of Health Team. The Office for CommunityHealth is looking for a full-time Lead Community Health Worker to work withadults during their inpatient stay at The University of New Mexico Hospital tohelp screen for and provide support for Social Determinates of Health.

Responsibilities:

• Assists the Program Management team supervise community health workers, to include work allocation, training, and problem resolution; evaluates performance and makes recommendations for personnel actions; motivates employees to achieve peak productivity and performance.

• Assists in the creation of training modules and continuing education curriculum for Community Health Workers.
  Train new Community Health Workers and provides ongoing staff development instruction.

• Core services provided by candidate may include coordination of cases with a multidisciplinary team to ensure patient Social Determinant of Health needs are being met during an inpatient hospital stay and evaluating the need for and connecting the patient with ancillary, wrap-around and supportive services

• May perform a range of administrative functions required to ensure efficient program operation

• Possession of a valid New Mexico driver's license is a requirement for this job.
• Must pass a pre-employment criminal background check.
• Fingerprinting, and subsequent clearance, may be required for this position depending on assignment.
• May be required to obtain a Community Support Worker (CSW) certification within six months of hire.
• May be required to hold a Certified Peer Support Worker (CPSW) certification if acting as a peer support worker.
• Employees who provide services or work in patient care or clinical areas are required to be in compliance with the University's influenza vaccination requirement.
• Successful candidate must submit to post-offer, pre-employment physical examination and medical history check.

High school diploma or GED; at least 5 years of experience directly related to the duties and responsibilities specified.

Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
