Position Description:
The Community Health Worker (CHW) is a patient advocate, helping patients and the community gain access to resources, including health insurance, food, housing, quality care, and health information. The CHW will work closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes. The CHC also works as an Outreach & Enrollment, Certified Application Counselor (CAC) for the Affordable Care Act, responsible to help patients of Eastport Health Care and non-patient residents of Washington County managing eligibility and enrollment in the Marketplace and the sliding fee program at Eastport Health Care.
All EHC employees are expected to engage and be active participants in quality improvement projects and efforts, risk identification and reporting, attend trainings and meetings as requested and contribute to overall team spirit and cooperation. Employee contributions in this regard are valued and necessary for the organization's successful care of our patients/clients.
Duties/Responsibilities:
Patient Navigation:
- Assist patients of the health center with special needs to apply for assistance in housing, Medicaid, Medicare applications and to help fill out other forms as required.
- Serves as a liaison between the client, physician, community agency, hospital, government agencies and insurance companies/brokers; and is actively involved in all areas of care coordination and case management.
- Assist patients/clients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible.
- Identify resources for clients to overcome barriers to care, such as transportation, housing, and childcare arrangements.
- Facilitate communication and coordinate services between providers.
- Continuously expand knowledge and understanding of community resources, services and programs provided.
- Maintains strictest confidentiality; adheres to all HIPAA guidelines/regulations
Outreach & Enrollment:
- Assist patients and non-patients of the health center to enroll in the Health Insurance Marketplace (Healthcare.gov) during open enrollment periods or when the person and their family qualifies for a special enrollment period due to life event s.
- Help the client determine if they have a qualifying life event.
- Upload proof of income or identify documentation to the Marketplace application.
- Help the client to make life event changes to their application with the Marketplace such as income changes, change in family size, change in jobs or address.
- Schedule outreach events to help educate the community to special enrollment events, open enrollment periods, or insurance literacy trainings (how does insurance work).
- Run ads in local papers, or radio public forums on open enrollments and special enrollment events.
- Track and report on assists, submits, and enrollment numbers to EHC monthly.
Minimum Entrance Requirements:
- Associate's Degree or equivalent in work history experience.
- Valid driver's license, clear driving record, and personal vehicle insurance coverage.
- Presentations, and Public Speaking.
- Ability to work with other employees, patients, and the general public.
- Able to utilize all office equipment such as scanners, lap tops, fax, and copy machines.
- Proficient in Windows, Outlook, Microsoft, and PDF files.
