Job Summary
At Syracuse Northeast Community Center (SNCC) we engage, grow, and connect neighbors, supporting them in moving from emergency to self-reliance and health.
The Community Health Worker leads Food Farmacy Program and Moral Reconation Therapy Program and collaborates with staff to coordinate services, fill gaps, educate, refer, and assess neighbors’ to promote community health and individual wellbeing.
The Community Health Worker will be an outgoing self-starter, compassionate, collaborative, organized, individual able to communicate with and advocate for neighbors’ health and wellbeing. The Community Health Workerdoes not need to be a medical professional.

Essential Job Duties and Responsibilities
Lead Food Farmacy program including Kitchen Additions, Kitchen Literacy, Community Garden, Food $ense, and Fresh & Healthy Produce Box distribution
Moral Reconation Training program counselor and facilitator
Community Health
perform and review intakes to ensure identified needs are met
administer quarterly Life Needs Assessments and evaluate results
monitor and utilize SNCC’s referral platform
develop new programs and ideas for individual and community health
Attend community meetings, events, and groups, promoting SNCC programs
Attend professional development and training as assigned
Other duties as assigned by Executive and Deputy Directors
Minimum Qualifications

An outgoing personality and the ability to interact well with others is a must
Syracuse city resident with strong community connections preferred
Relevant education and experience preferred
Excellent verbal, writing, listening, and observational skills required
Multi-lingual, multicultural abilities preferred
Must be self-directed, able to prioritize activities, and manage time efficiently
Must demonstrate a professional, welcoming, compassionate demeanor
Must be able to work an adjustable schedule
Certificates and Licenses
Valid and clean NYS driver's license
Able to pass criminal and DMV check upon hire
Equipment, Machines and Software Used
Regular use of computer, copier, phone, MS Office, Salesforce, and other software
Working Conditions and Physical Demands
Ability to travel throughout service area, occasional exposure to outside weather
Ability to work at a computer for extended periods and to attend meetings
