Job Title: Community/Hospital Liaison

Job Overview:
As a Community/Hospital Liaison, you will play a vital role in ensuring a smooth admissions process for all residents, serving as a bridge between residents, families, and the facility. This position involves close collaboration with hospital discharge planners, physicians, and various community partners, supporting our mission to provide quality care while fostering positive relationships in the community.
Key Responsibilities:
Administrative Duties:
- Maintain a daily log of bed vacancies and referrals.
- Obtain and verify residents’ insurance information promptly.
- Coordinate with residents and their families to complete the admissions process within 48 hours.
- Work closely with the Director of Admissions and communicate essential information swiftly.
- Gather and maintain necessary documents such as admission applications, insurance verification, and financial statements.
Marketing & Community Relations:
- Build and maintain relationships with physicians, social service departments, and hospital discharge planners.
- Promote the facility’s services through outreach and marketing campaigns.
- Represent the facility in the community by delivering speeches and presenting programs on aging and healthcare.
- Visit residents in hospitals or homes prior to transfer to ensure a smooth transition.
- Monitor and enhance the facility's reputation within the community.
Public Relations:
- Develop and manage relationships with community partners.
- Coordinate marketing events and campaigns that highlight the facility’s services.
- Address and resolve residents' complaints, ensuring proper documentation and follow-up.
Requirements:
- High school diploma or GED equivalent required.
- Minimum of 2 years of experience in healthcare admissions is preferred.
- Strong interpersonal and communication skills, with the ability to type efficiently.
Working Conditions:
- Office-based with some travel to hospitals and community locations.
- Interaction with residents, families, staff, and external partners is required.
- Participation in continuing education and staying updated on Medicare/Medicaid regulations is expected.
