Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states.
We are actively seeking Community Integration Counselors in the Sahuarita / Green Valley, AZ areas. This role provides individualized services designed to assist participants in managing and overcoming the difficulties confronted by an individual with a disability living in the community.
*This is a respite care position- attendant care provided in the family's home*
Responsibilities
- Support and encourage individuals to increase their ability to be as independent as possible
- Meet with waiver participants on a regular basis either in the individual’s residence, the community, or an office setting
- Collaborate with the Case Manager to develop and facilitate programs which meet the social, emotional, physical and personal needs of the individual
- Provide ongoing support for community integration efforts
- Document individual’s progress toward acquiring community integration
- Assist in building a support network between the individual and the community
- Ensure medical consultation forms are completed, by the physician, for each medical appointment attended
- Administer medications and ensures proper completion of necessary documentation
- Coordinates all leisure and volunteer opportunities
- Monitor the health and medical needs of individuals and immediately report any concerns
- Drive individuals to medical appointments and activities in the community
- Ensure program documentation and billable records are completed accurately and timely
- Additional tasks and responsibilities may be assigned
- Employee must cooperate with the license and department staff in any inspection, inquiry, or
investigation - Employment is contingent upon successful completion of checks of criminal background,
central registry, child abuse registry, and drug testing
Minimum Requirements:
- High School Diploma or GED
- Bachelor’s degree preferred
- Valid driver’s license with a satisfactory driving record
- Minimum two years of relevant experience supporting individuals with intellectual and developmental disabilities
- Ability to establish a comfortable and supportive relationship with individuals receiving supports
- Ability to exercise good judgment and remain calm in crisis situations
Working Conditions:
- Transport individuals utilizing your own vehicle or company provided vehicles
- Schedule may change due to business needs and may include evening and weekend hours
- May be required to be on-call in cooperation with other management staff
- Frequent lifting, stretching, and other physical exertion may be required
- May be required to lift or move 25+ pounds
- May be exposed to various medical conditions and communicable diseases
