As a Community Liaison, you will play a critical role in facilitating the referral process, expediting bedside visits, and cross-selling referrals to increase conversion rates, ultimately contributing to the growth of our company census. You will also assist in coordinating home and community referrals for the primary building(s). This role requires a strong understanding of the skilled nursing facility (SNF) business line, exceptional sales and relationship-building skills, excellent organization, multitasking abilities, and proficiency in utilizing various computer systems and hospital electronic health records (EHR).
Key Responsibilities:
* Referral Process Management: Expedite the referral process from hospital partners, ensuring a smooth transition for patients to our facilities.
* Bedside Visit Coordination: Facilitate and expedite bedside visits to assess patient needs and promote our services effectively.
* Cross-Selling Referrals: Identify opportunities to cross-sell referrals and services to maximize conversion rates and support the company's census goals.
* Home and Community Referrals: Assist with home or other community referrals as needed.
* Clinical and Payment Knowledge: Maintain a strong understanding of the SNF business line, including clinical and payment aspects, to effectively communicate and educate hospital partners, patients and families.
* Sales and Relationship Building: Build and maintain strong relationships with hospital partners, physicians, discharge planners, and other healthcare professionals to drive referrals. Strong internal relationships with facility staff and management teams are equally important.
* Multitasking: Handle multiple tasks and responsibilities efficiently to meet the demands of a high-volume hospital accounts environment.
* Technology Proficiency: Utilize various technology platforms and hospital EHR systems accurately and efficiently.
* Data Tracking: Maintain accurate records of interactions, referrals, and conversion rates for reporting purposes.
Qualifications:
* Bachelor's degree in a related field preferred.
* Previous experience in healthcare sales, Director of Admissions or Marketing, or a related role is highly desirable.
* Strong understanding of the skilled nursing facility (SNF) business line, including clinical and payment aspects.
* Excellent sales and relationship-building skills with a proven track record of success.
* Exceptional organizational and multitasking abilities.
* Proficiency in using various computer systems and technology platforms.
* Ability to learn and efficiently use hospital electronic health records (EHR).
* Strong communication skills, both written and verbal.
* Ability to work independently and as part of a team.
* Availability to travel to hospitals and healthcare facilities as needed.
* Valid driver's license and reliable transportation.
