Job Summary:
As a Community Liaison with Venza Care at Lynwood Rehabilitation and Healthcare Center, you will play a pivotal role in establishing relationships with community partners and healthcare professionals, generating new referrals, and ensuring seamless patient transitions to our facility. This position is essential to our mission of providing high-quality care and will work in close collaboration with the Admissions Director to ensure every referral is clinically and financially assessed.
Key Responsibilities:
- Relationship Building:
- Establish and nurture positive relationships with community case managers, social workers, discharge planners, and healthcare professionals.
- Actively engage with local communities to promote Venza Care's services.
- Represent Venza Care at events, health fairs, and networking meetings.
- Referral Management:
- Develop new referral sources and collaborate with the Admissions Director for clinical and financial review.
- Ensure a smooth and efficient admission process for patients and families.
- Admission Conversion:
- Convert referrals to admissions by coordinating communication between referral sources and Venza Care.
- Monitor and maximize conversion rates for a seamless patient experience.
- Marketing and Outreach:
- Execute marketing strategies to increase community awareness of Venza Care.
- Conduct presentations and provide educational resources to referral sources and potential patients.
- Communication and Coordination:
- Serve as the primary contact for referral sources and facilitate communication to ensure continuity of care.
- Collaborate closely with patients, families, and the Venza Care team for optimal admission experiences.
Qualifications:
- Previous experience in liaison, admissions, marketing, or sales within a healthcare setting.
- Understanding of skilled nursing and rehabilitation services.
- Strong interpersonal and communication skills, with a talent for rapport-building.
- Proven ability to manage multiple priorities independently in a fast-paced environment.
- Proficiency in Microsoft Office Suite and CRM software.
