Looking for a dynamic Community Living Manager to join the MTC team!
This is a full-time exempt position; salary in the mid-$45K range.
- Ensures compliance with all MTC policies and procedures, requirements of the Agency for Persons with Disabilities (APD) and other regulatory agencies.
- Maintains a working knowledge of federal, state, and local rules and requirements for applicable consumer services.
- Ensures collection of consumer data to complete program outcome reports quarterly.
- Attends mandatory meetings and training scheduled by the Area Office and/or Agency.
- Serves as the primary liaison between consumers, staff members, and other stakeholders (i.e. support coordinators, service providers, and family members) and collaborates with clients, families, and circle of support for best care opportunities.
- Responsible for managing Personal Support staff, Supported Living Coaches, including interviewing, hiring, motivating, conducting 90-day and annual performance reviews on staff, monitoring schedules, approving timesheets, and any disciplinary actions as needed.
- Ensures that coaches are managing their time well to maximize use of authorized service hours.
- Establish deadlines for the department as a whole.
- Ensures all required trainings for staff are completed within specified time requirements.
- Ensures adequate staff support to consumers during team member absences.
- Reviews consumer files to ensure required documentation is maintained in compliance with regulatory and auditing agencies.
- Maintains and updates all required information in case files for assigned consumers.
- Monitors and supports staff to ensure that all service delivery entries in iConnect systems are complete, accurate, and submitted in accordance to established deadlines.
- Performs editing and quality control of iConnect entries in preparation for billing.
- Keeps timely, detailed, and acceptable service logs.
- Accurately completes all required Florida Medicaid and APD/iBudget paperwork for each consumer.
- Develops implementation plans with consumer's input and provides needed support to achieve stated goals.
- Provides support and mentoring to consumers to assist them with achieving the goals and personal outcomes as stated in their support plans.
- Ensures healthy, safe, and comfortable living environment is being promoted by staff for the consumers.
- Ensures that the consumer's natural, supportive relationships within the community are maximized.
- Serve as an advocate by sharing potential interactions and associations with services and resources outside the field of intellectual differences.
- Ensures consumer satisfaction and choice surveys are completed for each consumer annually.
- Ensures consumers are developing and maintaining appropriate personal care skills and safety skills.
- Ensures community inclusion, medical appointments, prescriptions, transportation, third-party benefits, and other needs are being addressed for each consumer.
- Coordinates the notification of staff members and families during emergency evacuations.
- Maintain a collaborative work environment and work well with others.
- All other duties as assigned.
Bachelor's degree from an accredited college or university with a major in nursing, education, or social, behavioral or rehabilitative science required with at least one year of experience highly preferred. High school or GED diploma and four years of professional experience in working with persons with developmental disabilities may substitute for college yearly. Ability to manage people. Intermediate computer skills in MS Excel, MS Word, and MS Outlook. Must have the ability to proofread other's work, and excellent writing and communication skills. Excellent customer service. Ability to manage multiple projects simultaneously under limited direction while working under pressure to meet goals and deadlines. Problem-solving skills and critical thinking skills. Time management, highly organized and detail oriented is required. Must be a self-starter, and can be a contributing team member. Must have a people-first and service-oriented demeanor. Occasional night and weekend work may be required. Must comply with and pass Level II background screening (F.S. 435.04 and 409.907), Drug Screening, and FDOT/MVR check.
Post-employment: Must complete all required APD trainings (provided by MTC), including but not limited to HIPAA, Zero-tolerance, Direct Care Core Competency, Required Waiver Provider, iConnect, HIV/AIDS, CPR and First Aid.
